Summary
Critically ill patients require rapid access to definitive care and delays in treatment may lead to poor outcomes. This study will examine the effect of ambulance diversion on outcomes for patients with selected emergency conditions e.g. heart attack, stroke and major trauma who are and are not directly transferred to tertiary centres. Ultimately, this research aims is to improve patient outcomes and improve emergency service performance.

We aim to conduct a Phase 3 multi-centre, randomised, controlled trial to determine whether reducing oxygen administration to target a normal level as soon as possible following successful resuscitation from out-of-hospital cardiac arrest, compared to current practice of maintaining 100% oxygen, improves patient survival at hospital discharge.

Summary
Head injuries are a common in children. Even a seemingly minor incident may cause serious brain injury. Whilst head computed tomography (CT) identifies all important injuries there is a growing recognition that radiation from CTs increases the risk of fatal brain cancers, especially in younger children. This study of 10,000 children with head injuries aims to investigate the best way to accurately predict which children should undergo a head CT, and which children do not require one.
I am a clinician researcher in paediatric emergency medicine. My research focus is acute neurological problems and includes both high volume and rare but life threatening conditions in children. Award of an NHMRC Practitioner Fellowship will allow me to address major evidence gaps in the acute care of children presenting to the emergency department with head injuries and brain attacks (acute neurological dysfunction), and to translate the findings into widespread practice.

Summary
Expensive next generation blood tests offer the promise of more efficient detection of heart muscle damage in people with chest pain presenting to the Emergency Department. We aim to evaluate the treatment, management, cost and outcome at 1 year before and after these new blood tests were introduced in a national multi-hospital study.
Bell’s palsy is the sudden onset of one-sided facial weakness of unknown cause. Patients cannot close their eyes and mouths properly; this interferes with eating and speaking. In adults, there is conclusive evidence that a short course of prednisolone, a steroid, can increase the number of patients who recover completely. This study will investigate at 10 centres in Australia and New Zealand whether prednisolone should be used in children as well.
Out-of-hospital cardiac arrest is fatal without immediate resuscitation. Paramedic competency in resuscitation has been shown to influence cardiac arrest survival. Through my doctoral research I aim to investigate: how paramedic exposure to cardiac arrest can influence patient survival; the practices emergency medical services currently use to develop and maintain paramedic competency in resuscitation; and the confidence and perceived competency of paramedics responding to cardiac arrests.

By coordinating the Paediatric Emergency Medicine Centre of Research Excellence PREDICT will provide evidence to fill knowledge gaps in paediatric emergency care, develop better pathways to improve practice in the Emergency Department, and create a new ability to translate knowledge to improve care of children in Australasian emergency departments.
