Summary
Current heart attack treatments have focussed on re-opening the blocked coronary artery but despite this, many patients still suffer significant heart damage because of inadequate blood flow to the heart muscle due to damage to the small blood vessels - the microcirculation. This study seeks to identify heart attack patients with damage to the microcirculation and will conduct a randomised trial of clot busting medications to reduce microcirculation damage and to improve heart function.

We aim to develop and assess a new medical device for the treatment of diseased arteries. The new device uses an angioplasty balloon coated with a new drug to open narrowed arteries and will enable the arteries to remain open for longer periods and with less likelihood of unwanted side effects from the procedure.

Summary
While the focus in the treatment of coronary artery disease has primarily been on the large vessels in the heart, vessels of small calibre may also play a significant role in patients with angina, heart attack, and muscle injury related to a stenting procedure.This project aims elucidate the possible mechanisms which cause dysfunction in these small vessels. I also hope to find a treatment strategy which may benefit patients with small vessel problems and thus improve their overall outcome.

Heart attacks are currently treated with drugs to dissolve the clot in the coronary artery or by catheterisation with the aim of restoring blood flow to the ischaemic heart muscle. If restoration of blood flow occurs soon after the heart attack, the ischaemic region can recover completely. However if treatment is delayed, the ischaemic region may not recover. This project concerns the mechanisms that are involved in the myocardial damage which occurs after moderate periods of ischaemia. However, the downsides of these drugs are bleeding complications, which can result in death or disability. The consequences of these drug-associated bleeding compl ....The inhibition of platelets and the inhibition of coagulation factors are among the most widely used drugs in medicine and provide major benefits for numerous patients. Prevention and treatment of thrombosis, emboli, stroke and heart attack are examples of the many diseases where anti-platelet and anticoagulant drugs are administered. However, the downsides of these drugs are bleeding complications, which can result in death or disability. The consequences of these drug-associated bleeding complications are also a major financal burden for our health care system. Thus, progress towards therapeutic strategies with less bleeding complications is highly sought-after. The proposed project aims to generate new antibody-based agents for platelet inhibition. One group of these agents do only block platelets when they are activated. Furthermore, these agents allow an enrichment of potent inhibitors of coagulation factors at the site of the clot. Thus, these inhibitors should predominatly act at the site where they are needed. At the same time the overall concentration of inhibitors of coagulation factors can be kept low and the functions of non-activated platelet can be left intact. Summary
Arteries that have been surgically treated to remove fatty lesions that block blood flow frequently become re-blocked (restenosed), and drugs to prevent this re-blockage often have bad side-effects. We propose to prevent these complications by target-delivering the drugs directly and only to the required site using a single injection at the time of surgery. This will limit systemic side-effects, treatment costs and incidence of reblocking and bleeding complications.
