Summary
Diabetes in pregnancy provides exposure to high sugar levels before birth which can result in the child developing obesity, diabetes and cardiovascular disease later in life. This study aims to optimise the diagnosis and management of diabetes in pregnancy in the Northern Territory, thereby targeting the causes of chronic diseases from as early as possible in the life course, which we believe is a key step in closing the gap in life expectancy between Indigenous and non-Indigenous Australians.
Culturally Appropriate Diabetes Care In Mainstream General Practice For Urban Aboriginal And Torres Strait Islander People

Summary
Main study objectives are to determine the enablers and barriers for urban Indigenous people to access mainstream primary care services, in particular diabetes chronic care services. This knowledge of critical access and acceptability factors will guide the development of a culturally approirate diabetes care intervention for Indigenous patients. This adpated intervention, to be delivered in mainstream general practices, will be pilot-tested for cultural appropriateness in Indigenous patients.

Summary
Teleretinal imaging will be used to assess risk for diabetic retinopathy and cardiovascular disease and the value of this service will be used to implement a full suite of telehealth services using an already developed web-based open source software application that is made available to this project license free. The Telehealth service is expected to reduce the impact of chronic disease in Indigenous communities in a cost efficient manner and to reduce the health care disparity gap

Summary
This study aims to optimise the management of diabetes in pregnancy (both gestational diabetes and pre-existing type 2 diabetes) and post-partum follow-up of these high risk women in order to reduce the risk of future chronic disease among women and their children. The proposal involves scale-up of successful initiatives that we have developed as part of the NT DIP Partnership, scale-up within the Northern Territory (NT) and to Far North Queensland (FNQ).

Up to 30% of adult Aboriginal people have diabetes yet our knowledge of the causes and predictors of complications remain incomplete. We have established the PROPHECY Study to assess the levels of complications in Aboriginal people with diabetes; to understand the way that these complications occur, and identify what clinical, social and genetic factors could predict who will get those complications to guide clinical management and prevention.
I am an academic clinician who has a principal interest in all aspects of diabetes, especially treatment and complications, and who is also involved in studies of antimalarial pharmacology.

This study will evaluate outcomes and body composition in neonates born to mothers who have diabetes during their pregnancy in the Northern Territory. Babies born to mothers with diabetes may be at higher risk of complications during birth and later in life may have increased risk of obesity and diabetes, conditions which are particularly prevalent in the Indigenous population and contribute to high morbidity and mortality rates.
