Keeping my place in the community: achieving successful ageing-in-place for people with intellectual and developmental disabilities. This project, in partnership with aged care and disability agencies across two states, aims to examine ways the aged care, social and health needs of people with intellectual and developmental disabilities may be met in rural and urban settings. It will explore mechanisms that will lead to a better integration of the aged care and disability sectors.
Emerging from the shadows: the evaluation of intervention strategies to reduce social isolation amongst the aged. Social isolation imposes a high cost on affected individuals and the community at large. This project will determine what interventions work in addressing social isolation amongst the older population.
Addressing significant product safety knowledge gaps for older Australians . This project addresses significant gaps in contemporary knowledge of consumer product safety risks for older persons, with 25 years since the last Australian product safety research found older persons are at high risk of product-related injury/death. Products have evolved substantially and aged care models have changed in that time. A health economic model for the development and evaluation of innovations in aged care: an application to consumer directed care. This project will investigate the preferences, quality of life outcomes and cost effectiveness of a new consumer directed care approach to community aged care service delivery for older people. Summary
Modern chemotherapy treatments can result in damage to the peripheral nerves, resulting in a condition called peripheral neuropathy. This condition is characterised by a range of sensory and functional changes that can cause pain and reduced ability to perform daily activities. This project will test various non-pharmacological pain management measures to determine if they are effective in improving the quality of life of patients who experience this problem.

Summary
Rheumatic heart disease (RHD) is caused by an abnormal immune reaction to some bacterial infections. Although RHD is rare in developed countries, Indigenous Australians still live with the burden of RHD. The END RHD CRE will explore risk factors for RHD, prevention with antibiotics, management of RHD and the potential for vaccine development. Individuals and communities experiencing RHD are integral partners to this work. The CRE will establish a strategy for ending RHD in Australia.

Hospitals face high levels of emergency presentations and demand for inpatient care particularly for Aboriginal Australian people from remote communities. Readmissions lead to overcrowded emergency departments and poorer patient outcomes. We will evaluate the efficacy of a multidimensional case-based management intervention linking hospital and primary health in a regional Australian hospital with the aim of reducing hospital readmission and improving patient outcomes.
