Transfusion of donor blood is the traditional means of treating blood loss resulting from injury or during surgery. Consequently there is much interest in a range of techniques designed to reduce the need for transfusion of donor blood. Donor blood is in short supply and in the past there have been episodes of contamination by viruses that have led to hepatitis and AIDS. Consequently there is much interest in a range of techniques designed to reduce the need for transfusion of donor blood. One of the most popular is transfusion of patients' own blood (autologous transfusion). Despite the popularity of some of these techniques their true value is not really proven. They may be capable of diminishing the need for a transfusion of donor blood, but the long-term effects of this are not clear. In addition the preparation of autologous blood is a burden for overworked blood banks, and autologous blood may itself be associated with its own problems. The main aims of this study are to carry out reviews of the best quality trials of the various alternatives to transfusion of donor blood and to carry out surveys to find out what patients and doctors think. In a third phase of the project we will use the information gathered in the first two phases to design decision aids. These are comprehensive structured summaries of the available evidence to enable patients and their physicians to collaborate in making informed decisions that are likely to lead to the best outcomes for patients. The products of the research will be a number of reviews of 'best evidence' that appear in the International Cochrane Library, available to health professionals around the world. Summary
The effects of a high salt intake on blood pressure are well known. The damaging direct effects of salt on blood vessels is less well known. This study will clarify whether reducing salt to the amount recommended by Australian health authorities for 6 weeks has a beneficial effect on blood vessels. It builds on an earlier 2 week study which demonstrated that reducing salt intake had a beneficial effect on blood vessels. Too few blood platelets leads to fatal haemorrhage, and patients with low platelet counts require transfusions to prevent bleeding. We have recently discovered the key to keeping platelets alive, and now propose the critical experiments which will teach us how to manipulate it and allow platelets to live longer. Our team leads the world in this field. If successful we expect to improve blood bank platelet storage, and boost the supply of platelets available to patients in need of transfusion.

Summary
Blood platelets play a key role in blood clot formation, prevention of bleeding and are the principal elements contributing to thrombosis leading to heart attack and stroke. Numerous studies have defined pathways promoting platelet activity, however less is known about their negative regulation. In this grant we will examine the role for proteins, Dok2 and Dok1, in the negative regulation of platelets, hoping this leads to development of novel therapeutics for prevention of cardiac disease.

Summary
While spinal cord injury can cause devastating changes in the nervous system paralysis and loss of sensation relatively little is known about changes to the sympathetic nervous system. The sympathetic nervous system is intimately involved in the ongoing control of blood pressure, blood flow and temperature control. Loss of sympathetic control can occur following spinal cord injury. Interruption of descending pathways can result in partial or complete loss of sympathetic outflow from the thoracol ....While spinal cord injury can cause devastating changes in the nervous system paralysis and loss of sensation relatively little is known about changes to the sympathetic nervous system. The sympathetic nervous system is intimately involved in the ongoing control of blood pressure, blood flow and temperature control. Loss of sympathetic control can occur following spinal cord injury. Interruption of descending pathways can result in partial or complete loss of sympathetic outflow from the thoracolumbar segments. Complete decentralization can result in autonomic dysreflexia (autonomic hyperreflexia), in which sensory stimuli originating below the lesion evoke a reflex increase in sympathetic drive to the blood vessels, causing them to constrict. Because of this, blood pressure may rise suddenly and remain at such high levels that stroke and (occassionally) cardiac arrest may occur. This phenomenon, autonomic dysreflexia, is considered a medical emergency. The typical subjective signs of autonomic dysreflexia include a throbbing headache, tingling in the head or nasal congestion; sweating and flushing above the lesion are clinical signs that prompt medical staff to measure blood pressure and to locate the source of sensory irritation (usually a distended bladder or impacted colon, sometimes a pressure sore or ingrown toenail). Commonly, however, subclinical episodes go undetected, and this phenomenon of silent dysreflexia is of increasing concern. This project will develop means of assessing the integrity and state of the sympathetic nervous system below a lesion in patients with spinal cord injury and characterize the firing properties of reflexly activated sympathetic neurones.Read moreRead less
