This study introduces a new model of community care, based on case management, mentoring to improve self-efficacy and information systems to deliver information and closely monitor disease status. This will improve the quality of life of sufferers, develop healthcare networks and decrease healthcare utilisation.

Chlamydia pneumoniae is a relatively newly identified pathogen that is responsible for several respiratory conditions such as, pneumonia and chronic obstructive pulmonary disease and has been strongly linked to heart disesae. Serious disease is due to low grade chronic infections, but we do not understand how the bacterium causes these chronic diseases. This project will identify markers of chronic C. pneumoniae infections and relate these to the disease that results. There is an overwhelming burden of chronic disease in Indigenous Australians. In order to attempt to improve kidney disease in this high-risk population, it is vital that we are able to accurately measure kidney function. This study will provide evidence to accurately assess kidney function in Indigenous Australians. This will then enable development of appropriate clinical guidelines and more effective monitoring of future interventions to slow progression of kidney disease.

Aboriginal children living in remote areas such as central Australia experience unacceptably high rates of respiratory infection which can become chronic. A randomised controlled trial is being conducted to see if long-term azithromycin therapy reduces respiratory exacerbations in Aboriginal children with chronic lung disease. Concerns about the development of antibiotic resistance need to be addressed. This project aims to determine if such treatment is associated with antibiotic resistance.

Hepatitis C virus (HCV) infection is a major cause of liver disease and associated deaths in Australia. HCV infection leads to progressive liver failure and may be associated with the development of liver cancer. Currently there are an estimated 220,000 people in Australia living with HCV infection, and by 2020 it is estimated that this number will treble. There is now considerable evidence to indicate that the effect of HCV on the liver is due to ongoing immune activity and the build up of fat ....Hepatitis C virus (HCV) infection is a major cause of liver disease and associated deaths in Australia. HCV infection leads to progressive liver failure and may be associated with the development of liver cancer. Currently there are an estimated 220,000 people in Australia living with HCV infection, and by 2020 it is estimated that this number will treble. There is now considerable evidence to indicate that the effect of HCV on the liver is due to ongoing immune activity and the build up of fat (steatosis) in the liver. This results in the production of biochemical products that lead to tissue damage and to eventual destruction of the liver. Further evidence has recently emerged to suggest that the susceptibility to, and outcome of HCV infection may be influenced by genetic variation in the infected population. The chief investigators on this project have established the best characterised clinical cohort of HCV infected persons worldwide. Further, they have developed considerable expertise in the field of genetics, i.e. the analysis of genes that influence the host's response to an illness. Using this information and expertise, we propose in the present study to analyse in detail the host genetic factors that contribute to variations in the response to HCV, and its correlation with HCV-associated liver damage. Smoking -related chronic bronchitis and emphysema, otherwise known COPD, costs the healthcare system over $800 million per year. People continue to suffer even after they have given up smoking, and the treatments available result in only modest improvements. COPD is associated with a a defect of the scavenging cells in the lung, which normally clear away dying cells, and some of the proteins ivolved in this process. We will investigate whether supplementing these proteins will help.

More than three million Australians suffer from chronic pain, and there are few effective drugs available for treating this condition. A 2007 Access Economics Report estimated the economic burden of chronic pain in Australia at $34.3 billion. Acid-sensing ion channels (ASICs) are a recently discovered family of proteins that play a key role in sensing pain. The goal of this project is to develop potent blockers of these channels that can be used to treat patients suffering from persistent pain.

A specific course was developed in he US to help people self-manage. It has been available in Australia for 20 years through Arthritis Foundations and has become their core business. An important way that people with arthritis can deal with the disease is through using a variety of self-management behaviours and coping strategies as well as becoming well informed about the best available treatments. A specific course was developed in he US to help people self-manage. It has been available in Australia for 20 years through Arthritis Foundations and has become their core business. Treatment guidelines used by doctors to treat people with arthritis regularly recommend that patients should be referred to such courses. Although the course is widely distributed, the evidence scientific evidence regarding its effectiveness is patchy, and some overviews suggest it is not useful at all. Confusion exists regarding the value of the course. While treatment guidelines advise doctors to refer patients to the program by very few GPs, Rheumatologist or Orthopaedic surgeons do refer. The proposed study is a large controlled trial which will provide essential evidence to inform patients, doctors and policy makers on the benefits of the course. It will involve people with well defined moderate to severe arthritis who have consulted a surgeon or rheumatologist. People will be randomised to receive the intervention (two hours per week, six session course including an 'arthritis self-help' book) will be compared with people in a control group (who only receive the book without instruction). People will be followed for 1 year to see if the course improves quality of life, health behaviours, and whether less health care resources (ie attendance at doctors or less medication use) are used. The results of this study will be influential in determining government policy as the number of people with chronic diseases like arthritis is rapidly growing and the acute healthcare system, including hospitals, are poorly equipped to deal with this growing problem.Read moreRead less

COPD is very common and causes a great deal of debility and mortality in our community. COPD is also linked to an increased risk of lung cancer and carviovascular disease. It is estimated to cost Australians at least $800 million dollars per year in health related costs. Despite its importance, there is a limited understanding of how COPD develops and treatment options ....Chronic obstructive pulmonary disease (COPD) is a complex, chronic disease of the lungs principally caused by cigarette smoking. COPD is very common and causes a great deal of debility and mortality in our community. COPD is also linked to an increased risk of lung cancer and carviovascular disease. It is estimated to cost Australians at least $800 million dollars per year in health related costs. Despite its importance, there is a limited understanding of how COPD develops and treatment options are limited. We have identified large numbers of dying cells in the airways of people with COPD and we believe that these play a critical part in the cause and-or progression of the illness. This project will determine whether the increased rates of cell death are the result of the COPD process or part of the actual cause of the disease. This knowledge will enable us to address the urgent need to predict the risk of developing COPD in current and ex- smokers. Cells obtained from the lungs of healthy controls, current- ex smokers without COPD and current- ex smokers with COPD will be studied. The effects of current treatments for COPD on these cells as well as testing novel treatments will also be studied, paying particular attention to the effects on cell death.
