Unfortunately, these drugs can lead to serious heart problems which sometimes result in death, and the mechanisms behind this remain elusive. Cancers of the tonsil and base of tongue due to the human papilloma virus have a better prognosis than other head and neck cancers, but standard treatment can result in significant acute and late side effects. This trial aims to compare two types of chemotherapy and radiotherapy that are less intensive than standard treatment. The aim of the trial is to determine which treatment is associated with better quality of life and less side effects, while maintaining efficacy.

Summary
There is no effective way to identify all patients that will develop toxic side-effects during the course of their cancer treatment. Current pharmacogenetic testing is too narrow. This project aims to examine whole-genome profiles of patient blood to determine if risk markers of toxicity can be identified prior to beginning treatment. I will do this by comparing oesophageal cancer patients who go on to develop severe toxicity with those who only get mild treatment side-effects.

Cancer patients have large differences in the way their liver breaks down anticancer drugs, leading to a poor tumor response or severe side effects. This project will be with 2 cancer drugs which can cause severe side effects. Patients will have special liver Xrays to check their liver function and have blood taken to look at DNA or genes important in drug breakdown and action. These genes and imaging may help identify patients at risk of severe toxicity or poor response and optimise drug use.

Summary
Childhood chemotherapy often causes growth arrest, osteoporosis, and fractures in cancer patients and survivors. Using a rat model, this project will study how the most commonly used chemotherapy drug methotrexate causes bone growth defects and examine any protective effects of two natural-derived substances. This work will increase our knowledge on chemotherapy-induced bone growth defects, and will be useful for developing a preventative treatment.

Side-effects from anticancer drugs can be unpleasant, but may be deadly if a patient's genes are wrong for the drugs. A common chemotherapy drug, fluorouracil, can be very dangerous, but there's no simple way to predict this. We developed a simple test in which a small amount of a natural chemical is swallowed before chemotherapy. The results should allow the doctor to predict if a patient will suffer serious side effects on fluorouracil, so the dose can be reduced, or another drug tried.

Summary
Patient differences in clearance of anti-cancer drugs have a major impact on the success of chemotherapy. Benefit is lost if the drug is cleared too rapidly, while slow elimination causes toxicity. We will use well characterised mouse models that mirror the human situation to study the causes and effects of reduced drug metabolism in cancer. The data will guide future human studies that will result in improved diagnostic and therapeutic interventions to improve the tolerance of chemotherapy.
