Researchers in allied health and primary care are partnering with Northern Sydney Medicare Local and the NSW State Falls Program (Clinical Excellence Commission) to establish a multi-disciplinary pathway model for fall prevention. The aim is to establish integrated processes and pathways at the levels of practitioner, practice, and program to identify older people at risk of falls and engage a whole of primary care approach to fall prevention. This project will employ multi-methodologies.

Summary
Rheumatic heart disease (RHD) is caused by an abnormal immune reaction to some bacterial infections. Although RHD is rare in developed countries, Indigenous Australians still live with the burden of RHD. The END RHD CRE will explore risk factors for RHD, prevention with antibiotics, management of RHD and the potential for vaccine development. Individuals and communities experiencing RHD are integral partners to this work. The CRE will establish a strategy for ending RHD in Australia.

The local hospital emergency department (ED) is a prominent and highly utilised service in Australian communities but demand on EDs is increasing, resulting in significant service delays. In Australia, 48% of all suicides in 2000 were by 35-64 year olds; an additional 13% were by 65 years and over. This study will examine the last clinical contact older suicide victims had with health professionals in order to determine whether this contact offers an opportunity for intervention. A health economic model for the development and evaluation of innovations in aged care: an application to consumer directed care. This project will investigate the preferences, quality of life outcomes and cost effectiveness of a new consumer directed care approach to community aged care service delivery for older people. The applicability of a health economic model in the development and economic evaluation of innovations in aged care service delivery will be demonstrated.
Caring at end of life: understanding the nature and effect of informal community care networks for people dying at home. This project will provide an understanding of the function and purpose of caring networks for people at the end of their life, specifically for people who are dying at home. This understanding will enable the development of communities capacity in addition to informing a national health promotion approach to palliative care.
