Legionnaire's disease is a serious cause of community acquired pneumonia. We are studying the way the Legionella bacteria persist in the environment and cause disease. We have found that Legionella produces a specific protein that mimics the action of a human protein. Summary
Intestinal parasites are a global health problem. The World Health Organization recommends regular distribution of deworming drugs, targeting school aged children. This is effective for the children receiving the drugs but does not have an impact in the wider community. We aim to determine the best strategy for delivery of deworming drugs, to achieve the maximum benefit both for children and wider community, by directly comparing the benefits of a school-targeted vs a community-mass approach.

Summary
Malarial infection in pregnant women can lead to serious consequences for the baby including death, low birth weight and bacterial infection. Babies born in the community are more likely to die than babies born in hospital, therefore improving basic medical care for babies at the community level should be an important priority. We aim to evaluate the causes of death and severe illness in newborn infants in Papua New Guinea, and to determine community based strategies to improve these outcomes.

Summary
Respiratory illnesses are extremely common, but there is little information about patterns of infection in the community using modern diagnostic tests. Children have the highest rates of infection and transmit to all other age groups. We intend to recruit 138 newborns to monitor respiratory symptoms and collect specimens for testing in the first two years of life. This will allow us to document illnesses due to known and newly identified respiratory pathogens.

Aboriginal children have the highest rates of acute and chronic respiratory diseases ever documented. While the underlying cause is socioeconomic disadvantage, the features of poverty that result in disease are not clear. Our current understanding of important risk factors like overcrowding, malnutrition and smoke exposure cannot completely explain the excessively high rates of disease seen in rural and remote Aboriginal communities. Our current understanding of important risk factors like overcrowding, malnutrition and smoke exposure cannot completely explain the excessively high rates of disease seen in rural and remote Aboriginal communities. This randomised, double-blind, placebo-controlled clinical trial will measure the impact of antibiotic treatment on persistent nasal discharge and rates of transmission of bacterial respiratory pathogens. An innovative study design will test the efficacy of twice daily doses of amoxycillin on newly developed clinical and bacteriological outcome measures. Both the investigators and the Aboriginal community have considerable experience in conducting this type of research and are committed to using a cooperative model of clinical research to reduce disease burden. Ongoing training of Aboriginal health workers in the principles of clinical epidemiology is an essential component of this process. This study will make an important contribution to the medical literature. Despite the deaths of around 10,000 children children from acute respiratory infections each day, very little is known about how antibiotics affect the transmission of some of the principal causative organisms. The hypothesis that has the most evidence to support it is the 'hygiene hypothesis'. That is, that a reduction in early life infection among modern children leads to immune system dysfunction and thus an increase in immune disorders. The hypothesis that has the most evidence to support it is the 'hygiene hypothesis'. That is, that a reduction in early life infection among modern children leads to immune system dysfunction and thus an increase in immune disorders. Delayed child infection, particularly of Epstein-Barr Virus has been prospectively linked to multiple sclerosis (MS) risk. This project aims to document the role of herpes virus (EBV, HHV-6) infection in the onset of first demyelinating events, a precursor to MS. The strength of this proposal is that it lies within an existing study framework - the Ausimmune Study, allowing detailed exploration of related associations with latitude, early life infant contact and past sun exposure. The incidence of MS has doubled from 1.2-100,000 to 2.4-100,000 from 1961 to 1996 in Newcastle, Australia. Although incidence is low, the disease has a median age of first onset of 24 years and progresses to serious disability even with immunomodulatory therapy (50% will need assistance in walking within 13 years) thus the current prevalence of 1 per 1,000 adults in Tasmania and 0.8 per 1,000 in Newcastle represent a serious burden of morbidity.Read moreRead less

Streptococcus pneumoniae is an significant human pathogen which causes several diseases including pneumonia and meningitis. Treatment of infection involves the use of antibiotics such as penecillin, however, resistant strains are now emerging. This project will address the real need to develop new antibiotics targeting this organism. This is essentially a drug discovery project which exploits a novel means to target Streptococcus pneumoniae.

Before the introduction of antibiotics Staphylococcus aureus, the golden staph , was the major cause of infections in hospitals. Although the introduction of antibiotics helped control the organism it has gradually acquired resistance until strains have emerged which can only be treated with vancomycin. Consequently staphs have again emerged as a major hospital pathogen. The emergence of these multiply resistant strains corresponded to them acquiring methicillin resistance and consequently they ....Before the introduction of antibiotics Staphylococcus aureus, the golden staph , was the major cause of infections in hospitals. Although the introduction of antibiotics helped control the organism it has gradually acquired resistance until strains have emerged which can only be treated with vancomycin. Consequently staphs have again emerged as a major hospital pathogen. The emergence of these multiply resistant strains corresponded to them acquiring methicillin resistance and consequently they have come to be known as methicillin-resistant Staphylococcus aureus or MRSA. Soon after the emergence of MRSA the hospitals of Western Australia (WA) developed a policy to prevent introduced MRSA from becoming established in its hospitals. Although this has been successful the policy is now under threat with the emergence of MRSA in remote WA Aboriginal communities. Aboriginals in these communities have a large number of infections which are usually treated empirically. This can result in the selection of antibiotic resistant bacteria if they are present. Consequently, it is planned to regularly screen Aboriginal communities which are known to have a high prevalence of MRSA and recommend antibiotic prescribing which will not select for any resistant staphylococci carried by a person. This is possible because the community MRSA are still susceptible to some anti-staphylococcal drugs. If this program is shown to reduce the prevalence of MRSA in the communities then the program will be extended to other communities. Community MRSA are now being reported from other Australian states and it is planned to study these to see if they are related to the WA strains. The community isolates will be studied to assess their potential to acquire additional antibiotic resistances.
