Up to 50% of patients having surgery take regular medications and almost half of these patients have their medications stopped at the time of surgery. The interruption of their regular medications during this period exposes patients to associated complications. The project aims to improve the management of patients’ regular medications at the time of surgery, especially when they are fasting or nil by mouth, via a bundle of interventions that include education, reminders and audit/feedback.

The arterial switch operation is the surgery of choice for children born with transposition of the great arteries, a congenital heart defect where the main two vessels of the heart arise from wrong pumping chambers of the heart. There are very few studies looking at adults after this operation. We aim to study all patients who have had an arterial switch. Atrioventricular septal defects are a group of childhood heart diseases affecting all four chambers of the heart and two of the valves. These children do not normally survive past 30 years without surgery. Surgery offers a chance at life, but in the long term up to a quarter of children need further surgery. We are developing an Australian database to determine how best to reduce the risk of these complications, the burden on these children and the cost to society.

Early-onset knee osteoarthritis (OA) imparts a considerable burden on younger adults, by restricting physical activity, quality-of-life and work capacity. Treatment options for younger adults with early-onset OA are limited. I will investigate the immediate and medium-term effects of a commercially available brace on symptoms and physical function in people with early-onset knee OA. If beneficial, the brace may have capacity to improve the impact of early-onset knee OA in younger Australians.

Despite no clear evidence, transgender women are told to stop their feminising estrogen treatment several weeks before gender surgery, but this causes severe 'menopausal symptoms' with hot flushes, depression and distress compounding an already stressful preparation for and recovery from major surgery. This trial will establish if continuing estrogen gel around the time of surgery is safe and effective at improving 'menopausal symptoms'.
There is a link between maternal lipids and newborn weight as well as other adverse pregnancy outcomes. Maternal lipids _ serum level, dietary intake and change in maternal lipids during pregnancy may be important in determining not only immediate pregnancy and newborn outcomes but also longer term problems for mother and child. As yet, we do not know whether altering maternal lipids will change outcomes or how to influence lipid levels in practice or how best to approach this.
