Summary
The close contacts of people with multi-drug resistant tuberculosis (MDR-TB) have a high risk of developing the disease. The V-QUIN MDR-TB Trial will evaluate the effectiveness of an oral antibiotic (levofloxacin) in preventing drug resistant TB among infected household contacts of TB patients. Household contacts from 10 Provinces in Vietnam will be randomly allocated to receive six-months of either levofloxacin or a placebo, and then followed for two years to see if they develop tuberculosis.

Alcohol and drug treatment works: it improves health and reduces the social impact of alcohol and drug use. The treatment itself is not, however, the only variable that impacts on whether health outcomes are improved. The way in which governments fund, purchase and structure the treatment service system is also important. This study will empirically test the relationship between treatment outcome and the structures that governments put in place, providing new evidence to inform decision-making.

It is a chronically relapsing condition for which few, if any effective prevention and treatment strategies exist. Moreover, why an individual initiates and maintains heroin taking remains unclear. It is consistently associated with an adverse impact upon individual users, their families, and communities. It is a chronically relapsing condition for which few, if any effective prevention and treatment strategies exist. Moreover, why an individual initiates and maintains heroin taking remains unclear. Stress and negative emotions have a strong impact on heroin use. Stress may drive some individuals to start using heroin, stress increases the pleasurable effects of heroin and stress increases the aversive effects of heroin withdrawal. These effects will encourage addiction and discourage addicts from seeking treatment. Stress can also cause an otherwise drug-free individual to relapse to heroin addiction despite having been drug-free for some time. In this project we will study why stress has such a large impact on heroin addicts and heroin addiction. We will test the hypothesis that heroin use actually produces profound alterations in the neural network in the brain which controls responses to stress. This project uses a simple animal model of heroin addiction whereby rats are injected with morphine to study the regulation of several genes which are important in responding to stress. We will also study how this exposure and changes in gene expression alter neurobiological, cardiovascular, and behavioural responses to stress. This project will identify parts of the brain that are altered during heroin addiction, and will also identify why heroin addicts are more vulnerable to stress than the general population. Development Of A New High Throughput Screen For Drug Binding To HERG K+ Channels

Summary
Inadvertent drug block of hERG, a potassium channel in the heart, can cause cardiac arrhythmias and sudden cardiac death. Screening for hERG toxicity has become a major hurdle for development of new drugs. We will use a mutant hERG protein that has enhanced drug binding to develop a high throughput test for hERG toxicity. Identification of dangerous drugs early in the drug discovery process will save the pharmaceutical industry millions of dollars in the costs of brining new drugs to market.

Glycine Transporters regulate the concentration of glycine in the spinal cord and brain. It has been suggested that elevating glycine levels in these regions may be useful in treating pain and schizophrenia. This project will provide the basis for the development of new glycine transport inhibitors that may be used to treat these conditions.

Glucocorticoids are among the most effective drugs used in the treatment of many haematological malignancies, including leukaemia, lymphoma and multiple myeloma. However, the development of tumour cell resistance to these drugs remains a significant problem, and clinically relevant mechanisms of glucocorticoid resistance remain poorly understood. This project aims to define mechanisms of resistance to glucocorticoids and develop new drugs to reverse resistance.
Translational Research Initiatives In Acute Leukaemia

Summary
Recent research has focussed on molecular characterisation of high-risk acute leukaemia subtypes. This proposal will combine the power of genomic analysis, global analysis of protein kinases and stringent preclinical drug testing in order to improve the treatment of these high-risk acute leukaemia subtypes. Several innovative and interrelated projects within this Program will utilise a unique and clinically relevant experimental model to achieve their goals.
Snake envenoming is a neglected tropical disease and results in numerous deaths, psychological problems and economic burden. Antivenoms aren't readily available in resource poor countries. The CRE will investigate the effects of venom and antivenom to improve understanding of human envenoming. It will also develop tests for early diagnosis of envenoming and undertake clinical trials of antivenom. These studies will be translated back into clinical practice to improve treatment of snake bites.
Fracture Associated Premature Mortality; An International Consortium

Summary
Following an osteoporotic fracture there is an increased premature mortality compared with someone who has not fractured. There is also evidence that anti-osteoporosis medication reduces this premature mortality. This study will determine the size of the fracture-mortality relationship and the effect of treatment on this mortality for different fracture types and in different populations by integrating data from multiple international epidemiology studies of osteoporosis.
