Summary
Musculoskeletal conditions place a huge burden on the world’s population. Yet current trials in this field may not reflect priorities based upon this burden and few trials address well-recognised evidence-practice gaps. My fellowship will aim to transform the current ad hoc approach to Australian musculoskeletal clinical trials. It will identify the most critical unanswered questions, formulate a national research agenda, and identify best methods for optimising uptake of findings into practice.
Diabetes And Obesity: From Benchtop To Public Health

Summary
Obesity. Novel factors, not related to food intake or physical activity, will be explored as possible causes of obesity. The obesity paradox, in which overweight is associated with reduced mortality, will be examined. Diabetes treatment. Methods will be developed to predict which is the right drug for each individual patient. A study will test the use of nurse-led clinics. A novel insulin delivery mechanism will be developed. Novel causes and consequences of diabetes will be explored.
Prof. Simes will lead and develop research at the NHMRC Clinical Trials Centre in collaboration with major national and international cooperative groups. Research themes include integrating molecular science with trial design and embedding research routinely in health care. Projects include surgery for rectal cancer; novel targeted therapy in gastric cancer; aspirin to prevent colon cancer; placental transfusion for preterm babies; risk in heart disease; economic assessment of clinical trials.

Atherosclerosis and related cardiovascular disease (CVD) remains the single major cause of death and burden to health in Australia. Currently available drugs reduce the known risk factors for CVD (e.g., blood pressure, cholesterol). However, there is a need to develop novel drugs that protect in ways other than decreasing risk factors. Summary
The research proposed in this Fellowship is aimed at young people with emerging mental ill-health. The specific goals and targets are to formulate novel diagnoses that work; develop novel, personalised and safe psychosocial and biotherapies; and scale up well-evaluated 21st century models of mental health care.
I work on mitochondrial diseases, which are inherited disorders of metabolism that block conversion of food energy into chemical energy needed by our cells. We focus on understanding (i) the genetic basis of these disorders using approaches such as massively parallel sequencing, systems biology and experimental studies, and (ii) the detailed mechanisms of disease by studying cell lines from patients and animal models. We aim to develop better methods for diagnosis, treatment and prevention.
Lung disease is a major cause of death and disability world-wide. Tuberculosis was responsible for 1.4 million deaths in 2011. Asthma and chronic obstructive pulmonary disease (COPD) have been high priority diseases in Australia for many years. It was estimated that there were over 3 million deaths attributable to particulate air pollution in 2010. In this Fellowship I will undertake a series of projects designed to improve our understanding of these diseases and their causes.
