The diseases of ageing represent one of the major health challenges this century. Despite their increasing incidence, our understanding of the underlying causes is limited. A common feature is the accumulation of damaged proteins in cells and tissues. Damaged proteins are usually broken down by the cells and replaced, but in many age-related diseases this process fails. The most common source of protein damage is attack by oxygen-derived free radicals. These are by-products of our body's need for oxygen and can originate from atmospheric pollutants. Oxygen rusts metal, makes fat go rancid and can cause irreparable damage to proteins and other biological molecules. Free radical damage contributes to the development of many age-related diseases such as atherosclerosis and neurodegenerative diseases such as Alzheimer's disease. The accumulation of damaged proteins can cause cell death. Our knowledge of the mechanisms by which cells remove proteins damaged by oxygen and the reasons for their accumulation is limited. In this project we will use a novel technique we have developed to generate oxidised proteins in ageing cells. We will identify cellular mechanisms required for the efficient removal of damaged proteins and those mechanisms which fail in ageing cells. We will focus on a group of proteins which protect damaged proteins from aggregating and accumulating and we will examine how we can prevent the accumulation of oxidised proteins by stimulating the body s defence mechanisms. Since the population of Australia is ageing, diseases of ageing are going to consume an increasing amount of the national health budget. Summary
Australia is ageing rapidly but we still do not know whether the risk factors, such as health and lifestyle, that predict ill health in middle aged people, apply to people as they reach old age. This study of a large group of older men will examine the type and level of risk factors that apply to men aged beyond 75 years. It will not only determine rates of disability and mortality but also health service outcomes, including hospitalisation, and residential and community care usage.
Tailored Activities To Reduce Challenging Behaviours In Persons With Dementia Living At Home: The TAP Trial

Summary
Dementia is the greatest single contributor to the burden of disability for older people. Challenging behaviours are common and associated with reduced quality of life for the person with dementia and the caregiver. The Tailored Activities Program, delivered by occupational therapists, is novel. It targets the capacity of the person with dementia and provides a systematic way to match both capacity and personal interest to activity. We will conduct a randomized trial to test its effectiveness.
The aim is to make life safer for at-risk older people and to give them the capacity to participate in daily life activity. Transition from hospital to home and accidental falls are two events in which a lack of capacity to make adaptive changes or resume previous life activities can alter life course outcomes. I will engage in effectiveness trials of interventions and research to ensure the implementation of new knowledge into clinical and public health practice.

Summary
My research has focused on understanding how the process of cell division can result in different outcomes for adult blood stem cells. I am interested in determining the role of bone and blood vessels in the regulation of blood stem cells and in the development of blood diseases (myeloprolifertive disease). I will also determine the effects of changing the cell cycle with drugs to improve transplantation of blood stem cells.

Painful spinal or vertebral fractures are a substantial and growing public health problem and are a burden on the health care system. In Australia, 20-25% of women and 15-20% of men over the age of 50 will develop one or more fractures of their spine in their lifetime. Up to a half of these fractures will result in severe pain and disability. While the fractures generally heal within weeks or a few months, some are so painful that they require narcotic pain control, hospitalisation, and-or long- ....Painful spinal or vertebral fractures are a substantial and growing public health problem and are a burden on the health care system. In Australia, 20-25% of women and 15-20% of men over the age of 50 will develop one or more fractures of their spine in their lifetime. Up to a half of these fractures will result in severe pain and disability. While the fractures generally heal within weeks or a few months, some are so painful that they require narcotic pain control, hospitalisation, and-or long-term nursing home care. Other problems include chronic pain, spinal deformities, loss of height and mobility and restricted breathing. Vertebroplasty is a new procedure consisting of injection of a type of 'bone cement' into the vertebrae to mend the break. It is an exciting treatment because for some people, this results in an immediate and sustained improvement in pain. Although rare, complications such as rib fracture, cement leakage and fractures in other vertebrae do occur. One particular concern is that the mechanical changes to the spine caused by the bone cement may cause long-term complications such as an increased risk of future fractures of treated or adjacent vertebrae. So far, there is very little evidence of the efficacy and safety of this new technology. Importantly, it has not been compared with usual medical care for fractures. In medical research it can be difficult to properly evaluate an exciting and 'apparently' effective treatment that doctors and patients demand. The main aim of this project is to determine whether vertebroplasty is an effective and safe treatment compared to usual medical care. We have a unique opportunity to evaluate vertebroplasty through a strong collaborative effort between the 3 centres currently offering this procedure in Melbourne. If vertebroplasty can be demonstrated to be an effective, safe and cost-effective treatment for painful osteoporotic spinal fractures this will be a valuable addition to current treatment options.Read moreRead less
