    Optimising Benefits And Minimising Potential Harms Of Obesity Treatments

    Summary
    This work aims to find safe and more effective obesity treatments for the over 2.1 billion people worldwide with overweight or obesity. Severely energy restricted diets are currently the most effective dietary obesity treatment, but these are underutilized due to concerns about potential harms. This work will investigate using severe diets intermittently rather than continuously, as a means of enhancing their benefits of long-term weight/fat loss while also reducing potential harmful effects.

    Unhealthy diets are a leading cause of ill health. In contrast to our global leadership in tobacco control, Australia remains slow to use law as a tool to support healthy eating. This project will generate new insights on features of effective food policies to target excess sugar consumption. Using an innovative fusion of law and science, its findings will inform and accelerate current proposals to improve food labelling and tax sugary drinks to promote healthier Australian diets.

    Anorexia nervosa (AN) is a major public health problem. Nutritional rehabilitation programs exist but can be costly and protracted, and patients struggle to engage with these demanding therapies. The purpose of this study is to determine the efficacy of oxytocin nasal spray to improve outcomes in patients with AN participating in a nutritional rehabilitation program using a randomized placebo-controlled design. If shown to be effective, this will have national and international significance.

This project will examine feeding behaviour in rats fed palatable high fat diet, to examine when and how they eat. The contribution of brain chemicals implicated in addiction to this eating, and the impact of withdrawal of food will be examined.

    Anorexia nervosa (AN) is a serious mental illness that usually starts in adolescence and often runs a chronic course. With an estimated prevalence rate between 0.5% and 3.7% of women, and up to 50% remaining chronically ill, the illness poses a disproportionate burden on health and social services. AN has inpatient costs alone that exceed that for schizophrenia. Chronic AN has the highest mortality rate of any mental illness. Chronic AN patients are known for their ambivalence about engaging in .... Anorexia nervosa (AN) is a serious mental illness that usually starts in adolescence and often runs a chronic course. With an estimated prevalence rate between 0.5% and 3.7% of women, and up to 50% remaining chronically ill, the illness poses a disproportionate burden on health and social services. AN has inpatient costs alone that exceed that for schizophrenia. Chronic AN has the highest mortality rate of any mental illness. Chronic AN patients are known for their ambivalence about engaging in treatment and poor motivation to change their eating disorder behaviours. They often fail to respond to traditional treatments and develop a history of negative treatment experiences and repeated treatment failures. A new approach is needed to reduce both the personal suffering and the burden of the illness on social and medical services. To date, there has been little scientific investigation into the development of specific treatment for those patients with chronic AN. This study will trial a recently manualised therapy - non-specific supportive clinical management - which initial evidence suggests may hold promise for chronic AN because it offers a more indirect, motivationally-matched approach. This treatment will be compared to the establishment therapy Cognitive Behavioural Therapy. Patients will be randomly allocated to one of the two treatment conditions and will receive 40 sessions over 12 months. They will be thoroughly assessed prior to during and after they have completed treatment and followed up for 6 months. This is the worlds first trial of a psychological treatment for chronic AN; it is hoped the study will establish an effective treatment for this debilitating and expensive illness. Further, as the project aims to explore the core, but often over-looked, feature of AN - poor motivation for recovery - it will also be in a position to shed light on the deep psychological processes that maintain this illness.

    Bipolar disorder is often misdiagnosed as unipolar major depression, which can have disastrous clinical consequences. Emerging evidence indicates that individuals with bipolar disorder show particular dysfunctions within brain regions involved in processing reward. This research will use cutting-edge neuroscience methodologies to investigate reward processing in these two disorders, with the objective of identifying biological markers that help distinguish bipolar from unipolar depression.

    The burden associated with heroin dependence is undeniable. But little is known about the natural history and long-term course of heroin dependence; knowledge that is critical for informing the development of new treatment interventions, health care planning and service delivery. We propose to extend our study of 615 Australians with heroin dependence, recruited in 2001-2002, to 18-20 years follow-up to answer critical questions about the long-term impact of this condition.

    We will investigate the role of the orbitofrontal cortex (OFC) in decision-making, particularly the effect of hyperactivity in the medial vs. ventrolateral orbitofrontal cortex on decision-making. Hyperactivity in these structures has been linked to obsessive compulsive disorder and, in line with the distinct functions of the different regions of OFC, we develop and test a novel hypothesis as to the psychological and neural bases of the obsessions and compulsions distinctive to that disorder.
