Because of ongoing trauma, stress and disadvantage, Aboriginal and Torres Strait Islander (Indigenous) Australians face a greater risk of unhealthy drinking patterns and related harms than other Australians. Yet there is a shortage of specialist alcohol treatment services. This study examines an approach to supporting primary care services which target Indigenous Australians to identify and implement the best possible standard of diagnosis and treatment for unhealthy alcohol use.

We will investigate of the impacts of parental supply of alcohol between 13-18 years, on early adult alcohol use (19-23 years). We will follow an existing cohort of ~1,810 parent-child dyads, and assess the development of alcohol use, disorders, harms and aggression. We will investigate the impacts of: parental supply of sips vs drinks; supervised vs unsupervised supply; and modifying effects of contexts of supply and mediators (such as peer, child factors, etc.) controlling for confounders.

Alcohol causes harms to drinkers, and others around them, including spouses, children, family, friends, workmates and strangers. Australian and New Zealand work on alcohol’s harm to others is being replicated by the WHO in 7 countries (Chile, Nigeria, Sri Lanka, India, Thailand, Laos and Vietnam). We aim to analyse datasets from nine countries and compare the magnitude and patterning of problems across and within cultures – identifying opportunities for reducing harm from others’ drinking.
Neurosteroid Mediated Protection After Birth: Approaches For Maximising Protective Steroid Levels In The Neonatal Brain

Complications during pregnancy, birth asphyxia or premature birth can lead to neurological impairment in the newborn. Despite excellent neonatal care many of these babies go on to have serious handicaps. Neurosteroids are a group of steroids that regulate brain activity. These steroids protect brain cells from damage caused by an inadequate supply of oxygen by suppressing toxicity caused by excessive activity. We have shown that the levels of these protective steroids are remarkably high in the ....Complications during pregnancy, birth asphyxia or premature birth can lead to neurological impairment in the newborn. Despite excellent neonatal care many of these babies go on to have serious handicaps. Neurosteroids are a group of steroids that regulate brain activity. These steroids protect brain cells from damage caused by an inadequate supply of oxygen by suppressing toxicity caused by excessive activity. We have shown that the levels of these protective steroids are remarkably high in the fetal brain and levels rise further in response to fetal stress. The placenta contributes steroid precursors that help maintain these high neurosteroid levels. This placenta-fetal brain interaction comprises an internal mechanism that protects the fetal brain from adverse events during pregnancy. At birth, however, there is a dramatic decline in neurosteroid concentrations in the brain after the loss of the placental precursor supply. The fall in concentrations is even greater in animals that are born growth restricted. This suggests that newborns, particularly those from compromised pregnancies, are at increased risk of brain damage due to low neurosteroid levels. We believe that certain commonly used steroid therapies may also lower steroid levels in the brain and result in increased vulnerability to brain damage during birth or in the early neonatal period. Alternatively, we propose that replacement of neurosteroid precursors in the newborn may raise brain neurosteroid levels and protect against brain damage. In the proposed studies we will evaluate treatments that can raise the concentration of steroids and determine the best strategy for reducing brain injury following complications during pregnancy, at birth and during the early newborn period. The proposed study represents a unique opportunity to build on the NHMRC funded Climate Schools Combined (CSC) study, a world-first trial of a combined, internet-delivered school-based approach to preventing substance use, anxiety and depression. Having successfully implemented this trial and followed these students for 3 years, we propose to extend follow-up of this large trial cohort over the critical transition from secondary school into early adulthood.

Summary
A central belief in ageing research is that eating fewer calories prolongs life, and that the source of calories (carbohydrate, fat or protein) is irrelevant. However, a critical assessment indicates that this conclusion is premature. We will use recent techniques in nutrition to define for the first time in mammals the relationship between diet and ageing in a normal and a prematurely ageing strain of mice. The project will provide a novel nutritional approach for promoting healthy ageing.

Yet we have few models of well-implemented prevention programs for these common disorders. This project will assess the potential long-term benefits for young Australians of two school-based drug prevention programs (Climate Schools and Preventure) compared to drug education as usual. This world-first study will inform national and international policy by evaluating whether prevention programs delivered in Year 8 are effective in reducing alcohol and cannabis related harms, including risk of aggression and violence, over the high risk period during young adulthood (ages 18-20).

Preterm birth is the leading cause of death among newborns and the biggest contributor to disability among infants. Here we propose research to define the mechanism that controls the length of pregnancy and is disrupted in preterm birth. Specifically, we will determine what causes the repression of the labour-promoting inflammatory genes in the uterus during pregnancy and what activates them at labour. We will identify new targets for interventions to block or prevent preterm birth.
