Aboriginal Australians have higher death rates from cardiovascular disease (CVD) than other Australians, with rates in young and middle aged people increased 15-fold or more. The first step for selection of risk-reduction therapy in an individual is to assess his-her risk status and the benefit of intervention. Several risk assessment tools, incorporating traditional risk factors, have been developed from Western populations. They would be most appropriately applied to individuals who resemble the study sample; their validity in Aboriginal Australians has not been evaluated and no risk prediction formulae have been developed specifically for Aboriginal people. Recent reports suggest that a variety of other non-traditional risk factors also influence CVD risk, including markers of central fat deposition, inflammation, nutrition, alcohol use and early growth. Given the disadvantaged circumstances of many Aboriginal people, it is likely that some of the factors are influencing their CVD risk. The applicants have collected baseline risk factor data on volunteers in one Aboriginal community and followed their course, including CVD deaths and CVD hospitalisations, for more than a decade. From this baseline information, which goes beyond traditional risk markers, and from additional measures like birthweight, and markers of inflammation and nutrition assayed in retrieved serum, we will develop a variety of models for predicting a CVD risk in this group. We will also lay plans to evaluate these tools in other Aboriginal groups. A better understanding of the causes of CVD in this population is important to influence public health policy for CVD prevention. Thailand's disease patterns are changing. Some changes are good but others, like emerging diabetes, heart disease and injuries, are of concern. Development-related 'health transition' can be optimised if there is information on new causes of disease - immediate risk factors, and 'upstream' economic, cultural and environmental influences. South Australia is the fattest state in Australia. Limited health surveillance data suggests that Whyalla is fatter, less active, consumes more alcohol and has worse respiratory health than Adelaide. Obesity is linked to poorer respiratory health. Public concern over the effects of air pollution on the health of Whyalla families has prompted us to examine respiratory and metabolic health in Whyalla families in comparison to those in the north-west suburbs of Adelaide.

This project investigates whether the air pollution that was caused by the 2006-2007 Victorian bushfires, had an effect on the health of people living in Melbourne. Cardiovascular effects will be studied by looking at hospital admissions, out-of-hospital heart attacks and the associated death toll. As it is the expectation that bushfires will increase in the future it is important to determine the impact this may have. This research will inform appropriate public health strategies.

Understanding the mechanism by which contextual features are associated with risk for cardiovascular diseases (CVD) has implications for the development of efficient and targeted interventions. In three different studies we assess if the link between the context in which individuals live or work and risk for CVD (1) is indirect through effects on psychosocial factors and (2) varies as a function of such psychosocial factors in the general population and in populations at elevated risk.
