Summary
An important class of T cells known as T Follicular Helper cells (TFH) orchestrate the immune response so that we can produce antibodies to fight infection. The novel finding that our Lab made last year is that the molecule interleukin 21 (IL-21) is a growth factor for TFH. The findings we have made thus far could be used in a number of ways. For eg, IL-21could be used to expand numbers of TFH, using them to boost vaccination or natural defences against viruses, bacteria and tumour cells.

Summary
Despite the bacteria, the pneumococcus, being the most common reason why children die from pneumonia worldwide, the uptake of pneumococcal vaccine to prevent this disease has been slow, particularly in the Asia-Pacific region. I will work with Pacific governments, the Asia Development Bank Bank, UNICEF and other regional organisations, and health economists to lead the design of the implementation plan of PCV into Pacific island countries.

Vaccination has had a significant impact, but preventable infections continue to occur, perhaps due to delayed uptake of scheduled doses. For the first time, we will link vaccination and other health records to: provide accurate estimates of the impact of vaccination; identify reasons for delayed vaccination; and quantify the expected reduction in disease burden if vaccination timeliness was improved. The study will help determine who would benefit most from efforts to improve timeliness.

Over the last two decades much progress has been made towards the development of a malaria vaccine but none is yet available that is suitable for use in humans. Many parasite molecules have been identified that are considered potential components of a malaria vaccine and some of these have already reach the stage of being tested in early clinical trials. However, a major problem confronting the field of malaria vaccine development is finding the resources necessary to test the large number of antigens and antigen combinations that are considered of potential value. One way to gain information that will help to determine which antigens and antigen combinations should have priority for testing in clinical trials is to carry out vaccine trials in monkeys or mice using malaria parasites that infect these species. We will use Plasmodium chabaudi infections in the mouse to examine the ability of three antigens from the disease causing blood stages of the parasite to induce antibody responses that prevent the development of severe malaria. We will determine whether antigen combinations provide better protection than single antigens when mice are challenged with a variety of parasite strains. For these children, otitis media commences within the first few weeks of life and progresses to perforation of the ear drum in 60% of children by 12 months of age. Few children in remote Aboriginal communities have normal ear status or normal hearing and many have conductive hearing loss, affecting language development and education. Otitis media in these children results from infection with the bacteria Streptococcus pneumoniae, Haemophilus influenzae and-or Moraxella catarrhalis. Therefore the types of interventions we are using to combat the high prevalence of ear disease are antibiotic therapy, Streptococcus pneumoniae vaccination, and hygiene interventions. However we fear that the high load of bacteria carried by these children compromises the treatment and prevention strategies. The current proposal will test this hypothesis, and if found to be true, there will be implications for the management of otitis media. Development of a new way to vaccinate against influenza that will make standard vaccines 100 times more potent than conventional syringe injection. The Nanopatch is made from a silicon wafer, bristling with micro-nanoscale spikes. It painlessly deposits vaccine under the skin surface. Experiments in mice show that even a small vaccine payload delivered to the skin generates good immune responses. The Nanopatch vaccination system is expected to be ready for clinical trials within a few years.

One of the most promising treatments is functional communication training (FCT), which aims to reduce problem behaviours by improving the child's communication skills. Numerous studies have demonstrated the ef ....Approximately 40% of children with developmental disabilities have severe behaviour problems (e.g., aggression, self-injury, property destruction, and extreme tantrums) (Einfeld and Tonge, 1996). The care of a child with such problems represents a significant burden to parents and society. One of the most promising treatments is functional communication training (FCT), which aims to reduce problem behaviours by improving the child's communication skills. Numerous studies have demonstrated the effectiveness of FCT (Mirenda, 1997). Wacker et al. (1998) have shown that parents can be taught to use FCT, but these parents received individual consultations, which would seem to limit its practicality. Preliminary results from our current APEX grant suggest that parents can learn to implement this treatment in a group format, which is more practical from a public health perspective. However, it is currently unknown if group training is as effective as individual consultation. In the present application, we aim to compare these two ways of teaching parents how to use FCT. From a total sample of 36, the parents of 18 children will be randomly assigned to the group training format and 18 to the individual format. All parents will receive 12 hours of training on how to use FCT, either as part of a group of 3 families (mothers and-or fathers) in the clinic or on a one-to-one basis in the family home. Pre, post, and follow-up measures of problem behaviours will be used to address the research question: Is group training as effective as individual consultation? Vaccination during pregnancy has health benefits for mothers and their infants; however, it is a relatively new area of research, and the immediate and long-term consequences for children are currently not well understood. As part of this fellowship, I plan to conduct research into the long-term health impacts of vaccination during pregnancy. This fellowship will build my career as a perinatal epidemiologist and establish expertise in Australia related to vaccines given during pregnancy.
