Summary
The risk of drug resistance in tuberculosis (MDR TB) is thwarting control efforts and must be addressed. Potential strategies being considered are; treating MDR TB in the latent phase, using new short-course treatment for active TB and using new vaccines. My work develops simulation models to test strategies before they are implemented, to maximise impact and avoid costly or low-impact interventions. I will examine strategies in Australia and in high burden countries in Australia’s region.

Obsessive-compulsive disorder (OCD) is an incurable mental illness and current therapies only mitigate its symptoms for a portion of individuals. Thus, there is a need to identify the neural causes of OCD to develop personalised therapies. We will combine mathematical modelling, computer simulations, and clinical and neuroimaging data to develop the first model of OCD. Outcomes from this study will enable targeted OCD research and the discovery of brain mechanisms supporting treatment response.

Skin sores are highly prevalent in remote Australian Indigenous communities and can lead to invasive infections and rheumatic heart disease. We will develop mathematical models to understand the transmission of skin sores, allowing us to define the optimal extent (household, community, region), timing and triggers for interventions to interrupt transmission. This will guide public health policy in reducing the prevalence of skin sores and scabies, and their accompanying disease burden.

An important theme of the project is to consolidate our knowledge about how past exposure to non-pandemic influenza could provide short-lived protection against any new pandemic, and to explore the implications of this for prevention today. Another theme is to explore the severity of influenza during pandemics, and to identify social and medical factors that might reduce the dose of virus transmitted, or otherwise reduce the severity of infection. In developed countries, people who inject drugs (PWID) are the group at greatest risk of hepatitis C (HCV) infection but few PWID receive HCV treatment. With the advent of highly effective non-interferon based treatments HCV elimination, although ambitious, is now being seriously considered globally. This partnership grant will explore the feasibility of eliminating HCV transmission by enhancing HCV care and treatment for PWID in primary health care settings.

Class I cytokine receptors include around 30 receptors with diverse functions such as controlling metabolism and inflammation. Cytokine receptors are molecular switches on cells that receive signals from other cells and transmit this signal into the cell’s nucleus to control the regulation of genes. This project will determine the molecular mechanisms involved in class I cytokine receptors and use this knowledge to develop novel ways to modulate these receptors for clinical applications.

Diseases caused by the pneumococcus represent the largest cause of vaccine preventable death in the world today, mainly pneumonia and meningitis. In 2011, 16 developing countries will introduce pneumococcal conjugate vaccines, none in east Asia. Lack of research has been a major barrier to their use in the region. We have established an international centre of excellence in the field and we seek support to extend the capacity of this group and to transfer the technology to Vietnam.
