Breastfeeding has many advantages for the nutrition and health of infants and mothers. Perth Aboriginal Breast Feeding Study : Reasons for not Breast Feeding and for the Early Cessation of Breast Feeding. Breastfeeding has many advantages for the nutrition and health of infants and mothers. Recent research has shown the importance of longitudinal studies of breast feeding and the importance of family support systems, especially the attitude of the baby's father. In urban Aboriginal families the rate of breast feeding is lower and the duration is shorter than for those living in rural areas. There have been no longitudinal studies exploring the factors involved in the initiation and the continuation of breast feeding by Aboriginal women. A sample of 440 Aboriginal women will be recruited from Perth maternity hospitals. After an initial interview the women will be followed up at monthly intervals to find out their progress with breast feeding. In addition a number of focus group discussions will explore attitudes of Aboriginal women and men to breast feeding. The outcome of the studies will be information to feed back to the community through Aboriginal organisations. It will provide the basis for specifically targeted health promotion programs to maintain and to improve breast feeding rates in the Aboriginal community.

    Summary
    The mild iodine deficiency common among people living in South Eastern Australia is of sufficient concern for the government to order that all bread be fortified with low dose iodine. This fortification falls well short of the amount health workers think is required by pregnant women and this could prove harmful to their children. This trial will determine if a daily iodine tablet taken during pregnancy improves the development of children.

    Early Parenteral Nutrition Vs. Standard Care In The Critically Ill Patient: A Level I Randomised Controlled Trial.

    Nutritional support is accepted as a standard of care for the hospitalised patient however there is little agreement as to how it should be provided to the critically ill patient. Despite the fact that studies consistently link malnutrition to worse outcomes, the provision of nutritional support to the critically ill patient is highly variable. Although there is general agreement that it is best to feed critically ill patients via the gastrointestinal tract (stomach tube feeding), there is no ge .... Nutritional support is accepted as a standard of care for the hospitalised patient however there is little agreement as to how it should be provided to the critically ill patient. Despite the fact that studies consistently link malnutrition to worse outcomes, the provision of nutritional support to the critically ill patient is highly variable. Although there is general agreement that it is best to feed critically ill patients via the gastrointestinal tract (stomach tube feeding), there is no general agreement as to when intravenous artificial nutrition should be begun if a patient cannot tolerate a feeding tube. A recent systematic review of all available clinical trials suggests that if a critically ill patient cannot be fed by a stomach tube for at least 24 hours, they may benefit from intravenous artificial nutrition. This is not what currently happens under standard care. The purpose of this multi-centre randomised controlled trial is to determine if early intravenous nutrition saves lives. Because of the cost, and possible risk of increased infections, a study of this type is required before early intravenous nutrition could become a routine therapy in Australia. Informed consent to participate in the study will be obtained from next of kin, or directly from the patient themselves. The study will be managed at the Royal North Shore Hospital, University of Sydney and will include 26 collaborating ANZ hospitals over 18 months.

    Caloric Restriction, Ageing And The Liver Sinusoidal Endothelium: Mechanisms And Implications

    Old age is the major risk factor for many diseases yet the mechanism is unknown. We discovered age-related changes in the liver sinusoidal endothelial cell that provide a mechanism for the link between old age, lipid metabolism and vascular disease. The liver sinusoidal endothelial cell influences the transfer of substrates between the blood and liver cells, therefore changes in the liver sinusoidal endothelial cell affect liver function. We discovered major structural changes in the liver sinus .... Old age is the major risk factor for many diseases yet the mechanism is unknown. We discovered age-related changes in the liver sinusoidal endothelial cell that provide a mechanism for the link between old age, lipid metabolism and vascular disease. The liver sinusoidal endothelial cell influences the transfer of substrates between the blood and liver cells, therefore changes in the liver sinusoidal endothelial cell affect liver function. We discovered major structural changes in the liver sinusoidal endothelial cell in old age called pseudocapillarization, consisting of loss of pores, increased thickness and deposition of collagen and basal lamina. We showed that the loss of pores prevented the uptake by the liver of some lipoproteins, with implications for age-related changes in lipid metabolism and vascular disease. We have now found that caloric restriction delays pseudocapillarization. Caloric restriction is the only intervention known to increase maximal life span. This effect of caloric restriction is mediated by a protein called SIRT1 through actions on mitochondria and cell death. A naturally occurring agonist of SIRT1 called resveratrol has been found to increase longevity in yeast, worms and flies. We hypothesize that caloric restriction prevents age-related cardiovascular disease by delaying pseudocapillarization and hence maintaining hepatic metabolism of lipoproteins, particularly chylomicron remnants. We propose that caloric restriction will prevent age-related pseudocapillarization via its effects on the SIRT1 pathways and therefore pseudocapillarization will be delayed by resveratrol. Confirmation of these hypotheses will generate a unique target - pores in the liver sinusoidal endothelial cell - for the prevention of vascular disease in older people and provide a platform for the development of novel pharmacological agents such as resveratrol that act by maintaining the porosity of the liver sinusoidal endothelial cell.

    During pregnancy, omega-3 fats (like those in fish) are deposited in the brain of the developing baby. Women get these fats from the diet but Australian women typically eat a diet low in fish. Because research has linked high fish intake in pregnancy with better IQ in children, omega-3 oils are included in some pregnancy supplements. However their effect on child development is not clear. This project will test whether taking fish oil while pregnant improves a child's IQ.

    Vitamin D Deprivation In Early Life: Programming Of Vascular Function In Adulthood

    It is becoming increasing evident that appropriate nutrition in fetal-early life is important in programming the cardiovascular system of the offspring, influencing its function throughout life. Maternal deficiency in vitamin D is a recently-identified concern world-wide, including in Australian women. We have recently found that vitamin D deficiency in pregnant rats results in marked hypertension in the offspring, when only 7 weeks of age. This is associated with impaired endothelium-dependent .... It is becoming increasing evident that appropriate nutrition in fetal-early life is important in programming the cardiovascular system of the offspring, influencing its function throughout life. Maternal deficiency in vitamin D is a recently-identified concern world-wide, including in Australian women. We have recently found that vitamin D deficiency in pregnant rats results in marked hypertension in the offspring, when only 7 weeks of age. This is associated with impaired endothelium-dependent vasodilator function, increased smooth muscle tone and increased constriction to nerve stimulation. A combination of intracellular electrophysiological techniques and tension recordings will be used to investigate detailed mechanisms in arteries isolated from key vascular beds. In vivo studies will probe the role of vitamin D deficiency in the control of regional blood flow control, and its influence on the underlying regulatory mechanisms responsible for the cardiovascular dysfunction that we have observed. We will test whether the cardiovascular dysfunction in the offspring following vitamin D deficiency is reversible upon repletion, or is programmed and thus not reversible with repletion. Our early results suggest that the deleterious effects are not reversible. From this study we aim to be in a position of greater confidence from which to inform women as to the importance for their baby of ensuring adequate vitamin D repletion during pregnancy, to minimise risk of later cardiovascular disease.

    In Australia, one in four school-aged children is currently overweight and childhood obesity rates are continuing to rise. In the last five years it has become clear that rates of overweight and obesity are also rapidly increasing in pre-school age children. These are issues that are of concern to health officials as well as to parents. We understand that these rising rates in fatness relate to children's diet and activity patterns, and that these patterns, once established are likely to be patt .... In Australia, one in four school-aged children is currently overweight and childhood obesity rates are continuing to rise. In the last five years it has become clear that rates of overweight and obesity are also rapidly increasing in pre-school age children. These are issues that are of concern to health officials as well as to parents. We understand that these rising rates in fatness relate to children's diet and activity patterns, and that these patterns, once established are likely to be patterns for life. To date, there has been relatively little research regarding the chance to support parents to establish the eating and activity patterns that will protect their children from the very beginning of life. This project seeks to support first-time parents to establish healthy eating and activity behaviours in their children using the highly utilised Maternal and Child Health Centres first time parents' groups. We believe that these groups will provide a strongly supportive environment in which to provide parents with strategies and knowledge for promoting healthy eating and activity patterns for their children, and in turn, promote healthy weight gain. This project will assess the effectiveness of this intervention.

Although this has not been examined in human infants, new studies suggest that the iron stores of the mother in pregnancy are an important determinant of the baby's iron stores during the first year of life. It may be that the critical period where iron nutrition influences later dev .... Although this has not been examined in human infants, new studies suggest that the iron stores of the mother in pregnancy are an important determinant of the baby's iron stores during the first year of life. It may be that the critical period where iron nutrition influences later development is in fetal life (during pregnancy). We have the opportunity to test whether iron nutrition in fetal life influences childhood development by assessing the cognitive abilities of children who were in our earlier study of iron supplementation in pregnancy. Our aim is to assess the children at 4 years to determine if iron nutrition in pregnancy predicts development. This may change the dietary advice we give pregnant women to ensure they give their babies the best start in life.

    Postnatal depression strikes 12-17% of women in the year after giving birth. Postnatal depression can result in emotional, behavioural and developmental problems in children. Although there are many complex psychological and social factors involved, supplementing the diet with an omega-3 fat called DHA has been shown to alleviate the symptoms of depression. Enriching diets with DHA has also been associated with improvements in visual and neural abilities of young infants. Normally, DHA is obtain .... Although there are many complex psychological and social factors involved, supplementing the diet with an omega-3 fat called DHA has been shown to alleviate the symptoms of depression. Enriching diets with DHA has also been associated with improvements in visual and neural abilities of young infants. Normally, DHA is obtained through the mother's diet and is then passed onto the developing baby whilst in the womb. However, the amount of DHA that is delivered to the baby in pregnancy is high, and in most cases is far higher than most Australian mothers get in their diets. In this study we will test whether supplementing the diets of pregnant women with DHA results in women suffering less depressive symptoms and children with better development scores. If our study is successful, it will result in a simple and effective way to prevent postnatal depression and improve the developmental outcome of the children.
