Summary
Cardiovascular disease is a common and long-term health problem. Dr Redfern will continue to develop new and existing strategies for increasing uptake of and adherence to long-term behavioural change by people living with heart disease. The work will encompass policy development and media campaigns as well as trialling electronic communication systems such as text messaging and smart phone applications and new approaches including providing incentives for people who enact healthy behaviours.

This 4 year fellowship project engages with patients, caregivers, clinicians, researchers and policy makers, to establish core outcomes and outcome measures in dialysis and transplantation. This will ensure that patient-centred outcomes are consistently measured and reported in dialysis and transplantation trials and other forms of research.

Summary
People who have had a stroke are at increased risk of having another stroke. Secondary stroke prevention is a neglected area of post-stroke care and the uptake of research evidence is poor. Barriers to evidence integration can include problems with the evidence and difficulties at the health professional and/or consumer level. This research will seek to understand the barriers and develop strategies to reduce the gap between evidence and clinical practice in secondary stroke prevention.
