Summary
Prostate cancer is the leading cancer for Australian men but apart from getting older and having a family history of prostate cancer we do not know what causes it and nothing can be done to prevent it. This project follows up recent suggestions that some prostate cancer might be due to infections by bacteria and viruses. We will look for evidence of infection in blood samples and prostate tissue that we have collected from men with prostate cancer and from controls without prostate cancer.

The established risk factors for prostate cancer are only age, race and family history. We anticipate that this project will cast light on the role of hormones in prostate cancer and that we will identify new markers of risk of prostate cancer and markers of disease aggressiveness. These outcomes will help us identifying men who are at risk for prostate cancer to target screening and surveillance, and plan prevention strategies. In 1990 we set up a long-term study of diet and health. The aim was to measure diet and other risk factors in healthy people in order to see how they might affect future development of cancer. To do this we recruited 41,500 people aged 40 to 69, measured what they ate and drank, and collected information on other aspects of lifestyle, medical history, and family history of common diseases. All had height and weight and blood pressure measured and gave a blood sample. People were selected so that ....In 1990 we set up a long-term study of diet and health. The aim was to measure diet and other risk factors in healthy people in order to see how they might affect future development of cancer. To do this we recruited 41,500 people aged 40 to 69, measured what they ate and drank, and collected information on other aspects of lifestyle, medical history, and family history of common diseases. All had height and weight and blood pressure measured and gave a blood sample. People were selected so that men and women and migrants from Italy and Greece would be included. In this way we could widen the range of dietary habits, other lifestyle factors and genetic variation (measured in DNA from blood). Since then participants have completed another questionnaire and instances of disease have been noted from self reports and from examining medical records. We want to analyse data from 700 men in the study who have developed prostate cancer (PC). First we will analyse data collected on all 17,000 men (collected when joining the study, and at follow up). Next we will use data from only the 700 men with PC and 1400 men who have not developed PC. This study will focus on measuring substances in the blood. We want to measure a range of fats, vitamins, antioxidants and phytoestrogens, as well as male sex hormones and related substances. In the DNA from the blood we plan to measure variations in genes that influence how male sex hormones and other growth factors important in the prostate are produced and used. We will then be able to estimate what affect these factors have on the risk of getting PC. We will also be able to see if any of them act together to make the risk of PC much higher in certain men. This work should identify what lifestyle factors could reduce the risk of PC. It should also identify what genetic variations are associated with increased risk of PC and thus identify a sub group of men who might benefit from early medical attention or from changes in lifestyle.Read moreRead less

In Phase I of the Fremantle Diabetes Study (FDS), valuable and detailed data on a wide range of subjects were obtained from a community-based patient cohort between 1993 and 2001. There is a large body of evidence that the nature and treatment of diabetes in Australia is changing rapidly. In order to provide up to date information to health care providers and government agencies, to confirm observations made in FDS I and to venture into new research areas, Phase II will be conducted.

This project forms the central component of a larger set of studies which investigate competing psychiatric, immunological and infective models of the causes of a number of chronic fatigue syndromes, including post-infective fatigue. The study takes place in western NSW where certain viral illnesses (Glandular Fever, Ross River Virus) and a non-viral infection (QF) are common and have been associated with prolonged fatigue states. The study follows patients from laboratory-documented infections ....This project forms the central component of a larger set of studies which investigate competing psychiatric, immunological and infective models of the causes of a number of chronic fatigue syndromes, including post-infective fatigue. The study takes place in western NSW where certain viral illnesses (Glandular Fever, Ross River Virus) and a non-viral infection (QF) are common and have been associated with prolonged fatigue states. The study follows patients from laboratory-documented infections with appropriate infective, immunological and psychological measures throughout the course of their acute illness, the early recovery period and for the next 12 months. These patients are compared with people who present to their doctor with other forms of medically-unexplained fatigue. Very few previous studies have used an appropriate prospective design and followed patients with documented illness from the onset through to the development of specific forms of chronic fatigue. Further, the study is unique in terms of the range of viral and non-viral agents being investigated. It relies on the combined psychiatric, immunological and infective disease expertise of a group of researchers with an international reputation for the successful completion of such multidisciplinary projects. The initial phase of the study has demonstrated that the research team has the capacity to complete this project. Initial results have already demonstrated the potential roles of psychological and immunological factors in causing some cases of prolonged fatigue. Further, the initial results indicate that two key symptom sets (fatigue, psychological distress) can be adequately measured during the recovery phase and are predicted by differing psychological factors. The study will result in the identification of different psychiatric risk factors to chronic fatigue, assist development of clear diagnostic guidelines for post-infective fatigue and guide relevant aetiological and treatment research.Read moreRead less

Summary
Indigenous people world-wide have higher rates of kidney failure than non-Indigenous people. Aboriginal Australians have the highest rates of kidney failure in the world, especially in those living in remote areas. The reasons for this are complex, and include well-known environmental risk factors that contribute to many diseases in Aboriginal people - socio-economic disadvantage, higher rates of infection, smoking, alcohol abuse and poor nutrition. There are also biological risk factors more sp ....Indigenous people world-wide have higher rates of kidney failure than non-Indigenous people. The reasons for this are complex, and include well-known environmental risk factors that contribute to many diseases in Aboriginal people - socio-economic disadvantage, higher rates of infection, smoking, alcohol abuse and poor nutrition. There are also biological risk factors more specific to kidney disease such as low birth weight babies, reduced kidney volume, female sex, family history of kidney disease, genetic influences, over and under-nutrition and high blood pressure. Many of these risks may take effect in childhood, resulting in silent kidney damage that may become chronic in adulthood when diabetes and other influences take effect. In order to clarify the degree of risk early influences have on Aboriginal kidney disease before adult confounders complicate the picture, a unique study of early signs of kidney disease in outwardly healthy Aboriginal children was planned. These school children come from different locations across NSW, and have a non-Aboriginal comparator group. The first primary aims are complete: To determine: 1. Rates of blood and protein in the urine, and high blood pressure in Aboriginal as compared to non-Aboriginal children. These are the early markers, or antecedents of kidney disease; 2. If these antecedents differ over urban, coastal, rural and remote regions, and socio-economic areas; 3. Any association between antecedents and other risk factors such as age, gender, birth weight and growth; Secondary aims are currently underway: To determine: 1. The natural history of these antecedents of kidney disease by following these children for a further 4 years; 2. Chronic Respiratory Diseases (CRDs) are a major public health problem. It is known that CRDs change over time but we have no information on causes of these changes. Some childhood asthmatics continue to have asthma as adults and-or develop Chronic Obstructive Pulmonary Disease (COPD) while others are free of any adult CRD. Some of those who do not have childhood asthma develop asthma and-or COPD as adults while the others remain free of CRDs from childhood to adulthood. To investigate risk facto ....Chronic Respiratory Diseases (CRDs) are a major public health problem. It is known that CRDs change over time but we have no information on causes of these changes. Some childhood asthmatics continue to have asthma as adults and-or develop Chronic Obstructive Pulmonary Disease (COPD) while others are free of any adult CRD. Some of those who do not have childhood asthma develop asthma and-or COPD as adults while the others remain free of CRDs from childhood to adulthood. To investigate risk factors for these changes, following up siblings over time is a powerful tool. As siblings share the childhood environment but not the adult environment, it helps to disentangle childhood environment, adult environment and genetic factors. The Tasmanian Asthma Study (TAS) is amongst worlds' major longitudinal respiratory studies and it is unique because it was conceived as a family study, with all the family members and the family environment being surveyed. TAS commenced in 1968 by investigating 8,585 school children born in 1961 (referred to as probands), their parents (16,267) and siblings (21,044). By the end of 2006, we will have completed the 37-year follow-up of the TAS probands, which focuses on non-genetic risk factors for middle-age CRDs. This follow-up together with baseline data now provides a unique opportunity for conducting a sibling study, which can concurrently examine genes, childhood environment and adult environment for change in CRDs. Also, it will provide a platform for future studies to investigate the progression of CRDs in this family cohort. Therefore, we now seek funding to extend the current follow-up to include the siblings. This will be the world's only population-based respiratory sibling study that spans childhood to adulthood. This study will follow up over 12,000 men aged 65-83 years who have previously completed a questionnaire about their medical, social, family and occupational history and lifestyle and have undergone a simple physical examination (height, weight, waist, girth at the hips and blood pressure). The first aim is to determine which of these factors predicts serious ill-health from heart disease, stroke and certain cancers in old age. In addition, we will test the hypothesis that risk factors for heart ....This study will follow up over 12,000 men aged 65-83 years who have previously completed a questionnaire about their medical, social, family and occupational history and lifestyle and have undergone a simple physical examination (height, weight, waist, girth at the hips and blood pressure). The first aim is to determine which of these factors predicts serious ill-health from heart disease, stroke and certain cancers in old age. In addition, we will test the hypothesis that risk factors for heart disease also predict the development of dementia because it seems that part of dementia in some patients is explained by their having suffered multiple small strokes. Finally, we will examine the relationship between development of dementia and previous major surgery as there is growing evidence that major surgical operations in older patients can be followed by confusion and impairment of thinking that may never recover fully.
