Summary
Our study investigates cancer risk following exposure to low doses of ionising radiation from medical procedures. Our first major paper linked over 800,000 CT exposures to cancer outcomes in a cohort of almost 11 million young Australians, and found that CT exposure predicted an increased incidence of leukaemia and most solid cancers. In our ongoing work we will incorporate nuclear medicine and other diagnostic x-rays, and estimate radiation dose for individual procedures and to specific organs.
Fighting Epidermal Skin Cancers By Targeting Epidermal Clones That Accumulate Mutations

Summary
Common skin cancers such as basal and squamous cell carcinomas (BCC and SCC) are by far the most frequent cancer worldwide and require over a million interventions per year in Australia. This project will identify the skin cells that are most susceptible to give rise to cancer if excessively exposed to the sun and explores ways to prevent cancer formation. This will inform on new strategies to prevent new skin cancer development.

Summary
Despite 100 years of research there is uncertainty about effects of low dose radiation from background and medical X-rays. We will measure the incidence of cancer in Australians exposed to CT scans (medical X-rays) as children between 1985 and 2005. Our results, from follow-up to 2009, will show whether there is a small but signicantly increased risk of cancer and guide further improvement in radiation safety standards if these prove to be necessary.
Biophysical identification of natural human antibody targets. A natural human antibody, PAT-SM6, isolated using technology developed by the partner organisation (Patrys), offers promise as a therapy to reduce mortalities due to cancer, the leading cause of death in Australia. The novelty of the approach pioneered by Patrys is the direct production of human antibodies which avoids undesirable side effects associated with the use of antibodies containing non-human components. Summary
The weakest link in radiotherapy is defining treatment volumes (contouring). Lack of accuracy and consistency in clinical trial contouring has been shown to result in reduced patient outcomes. Manual review of contouring is resource intensive, expensive and for advanced treatments unachievable in a timely fashion. We will assess an automated approach to contouring assessment using 4 clinical trial datasets, changing practice for future studies and enabling consistent assessment in the clinic.

This study is needed to determine whether a common clinical practice long-term antibiotic treatment for children following urinary tract infection (UTI) - is safe and effective in preventing further UTI and if so, whether all appropriate children are being treated. UTI will affect about 10% of Australian children by high school age (88,000 children per year). Because UTI may damage the kidneys, the management priority for children with UTI has been prevention of further infection. Currently this ....This study is needed to determine whether a common clinical practice long-term antibiotic treatment for children following urinary tract infection (UTI) - is safe and effective in preventing further UTI and if so, whether all appropriate children are being treated. UTI will affect about 10% of Australian children by high school age (88,000 children per year). Because UTI may damage the kidneys, the management priority for children with UTI has been prevention of further infection. Currently this means the identification of children thought to be most at risk of recurrent UTI by renal tract imaging. Those found to have reflux of urine from the bladder to the kidney (present in about 30% of those with UTI) are then placed on antibiotics fro 2-5 years. Unfortunately there has never been a properly designed trial to test whether antibiotics do really prevent UTI and if so, whether children with reflux are the appropriate and only group requiring treatment. Long term antibiotics may in fact do more harm than good because of side effects like skin, bowel and blood problems and because resistant bacteria may develop. The design of this study involves the random allocation of placebo or antibiotic (cotrimoxazole, the usual antibiotic given in this case) to about 800 children after their first symptomatic UTI. These children are treated and followed for one year to determine the rate of futher UTI in both groups. Any difference in outcome between the two groups of children will be because of the antibiotic treatment. This study may prove long-term antibiotics are ineffective and therefore should not be routinely used. In this case investigation of children to detect vesicoureteric reflux would serve little purpose and should be abandoned. Alternatively antibiotic treatment may be shown as effective treatment for preventing further UTI and in this case the study will clearly identify those children who will benefit.Read moreRead less

Sexual dysfunction is one of the most common and distressing side effects of prostate cancer. Despite being a critical survivorship care issue, there is a clear gap in knowledge surrounding the optimal treatment of sexual dysfunction in men with prostate cancer. ASPREE is a large, phase 3 clinical study of health participants over the age of 70 years who have been randomized to either continuous low dose aspirin or placebo for an average of 5 years. This grant is concerned with collecting long-term follow-up for an additional 5 years, especially for evidence of colorectal cancer (CRC) as well as the exploration of potential mechanisms of action by which aspirin may prevent the development of CRC.
