Summary
Antibiotic resistance is a worldwide crisis. It means antibiotics no longer work. Reducing their use is critical. Acute respiratory infections (eg ear infections, sore throats, cough) are a target for reducing use as they are the most common reason that general practitioners (GPs) prescribe antibiotics, despite being usually not needed. This trial will test if decision aids reduce antibiotic use, by helping GPs and patients to discuss their benefits and harms and jointly decide about their use.

Chronic diseases are the leading cause of death and disability in Australia. Since 1999, patients in Australia have been able to access a variety of comprehensive disease management programs subsidised through Medicare. We will use comprehensive linked data from survivors of stroke registered in the Australian Stroke Clinical Registry to provide the first robust evaluation of the effectiveness of these enhanced models of primary care in patients with chronic diseases using stroke as an example.

This study aims to develop and test a novel model of shared care for the follow-up of men with prostate cancer. Men treated for prostate cancer experience distressing and ongoing side-effects of treatment and significant psychological issues. Current models of care fail to address adequately these issues. We will study the use of a new technology, retrospective continuous glucose monitoring (r-CGM), to help achieve glucose targets for people with type 2 diabetes (T2D) in General Practice (GP). This is important because controlling glucose levels improves disease outcomes and because T2D is mostly managed in GP where the majority of people are out of glucose target levels and GPs and patients currently don’t have a simple effective method for monitoring blood glucose levels to guide treatment.

The Target-D Study uses a novel clinical prediction tool to test a new approach to depression care in general practice based upon sub-grouping patients into low, medium and high risk of ongoing depression. Participants will be randomly allocated to targeted treatments based upon their risk profile or to usual general practice care. We will measure whether the new approach results in greater improvements in depressive symptoms, quality of life and functioning and whether there are cost benefits.

Diamond began in 2005 to follow over 500 people with depressive symptoms to document and describe the nature and course of depression and its’ management over a decade of a participant’s life. Our findings will help us to understand the factors associated with depression recovery, relapse and persistence. We will inform clinical practice by developing a tool to assist GPs to identify those at risk of persistent and disabling depression, to enable better treatments.

Lung cancer is the leading cause of cancer deaths in Australia and the second commonest cause of all deaths in men. Earlier, timely diagnosis could be a critical factor in improving lung cancer outcomes as many people present with late stage disease having had symptoms for a long time. This trial tests a novel nurse-led intervention in general practice aimed at promoting earlier consulting by people at higher risk of lung cancer when they develop chest symptoms.
