Although osteoporosis affects the elderly, the most opportune time to prevent osteoporosis may be during childhood. Physical activity is a lifestyle factor known to affect the strength of the skeleton. Bone density is commonly used as a measure of bone strength because it is easily measured and is related to the breaking strength of bones. However changes in the shape of bones can also affect bone strength with or without an increase in bone density. Changes in bone shape in response to exercise in children or adults have rarely been investigated, and little is known about the effects of exercise on bone shape during different stages of growth. This study is the first to investigate how exercise during childhood may affect bone strength by changing bone shape. 45 elite female tennis players aged between 6 to 18 years have completed two years of this study. 90 novice and competitive male tennis players aged between 6 to 20 years and 60 healthy age matched controls will be asked to participate in this study. Measurements will be made annually for three years. The bone shape and density of the dominant and non-dominant arms of the players will be compared with the children who don't play tennis - comparisons will also be made between i) different stages of puberty and ii) girls and boys. The findings of this study are important because the lifestyle of children today may predispose them to a greater risk of osteoporosis late in life. Physical activity may be the most important modifiable protective factor against fragile bones in old age. Summary
Millions of scripts are filled for treatment of osteoporosis. However, there is no way of knowing if these drugs are right for these individuals, if it improves bone strength or are actually doing harm. Bone density measurement is of limited value. We have developed a new analysis method that measures changes in bone structure that tell us if the treatment is or is not working so alternative treatment can be used. The aim of this study is to test this new method.

Refeeding program guidelines have been established and implemented when children are assessed as being malnourished. Recent studies on the effect of these important programs throughout the world have suggested they may not have been as successful as wished. Indeed, even though many of the subjects are the urban poor, it has been found that many are developing diabetes in their twenties. This is associated with an increase in abdominal fatness, out of proportion to the rest of the body's size and shape. The increase in diabetes in this group is most worrying and a major public health problem. It is not known whether the diabetes and abdominal fatness are caused by the refeeding programs or whether they are due to the fact that the body's metabolism was programmed during pregnancy. Perhaps the infants were malnourished whilst they were developing during pregnancy and metabolism was changed so they could store fat easily and not use glucose as readily in the rest of the body. This study is designed to show whether there are metabolic changes which predispose to diabetes and fatness present at birth in those who are malnourished or whether the refeeeding program designed to help them grow is the cause of these later problems.Read moreRead less

Of concern is the fact that VLED induce adaptive responses that can inhibit loss of, and promote regain of, abdominal fat while decreasing lean body mass, muscle strength and bone density. This project will therefore determine whether VLEDs could have negative effects on body composition that increase the risk of metabolic disease, sarcopenia or osteoporosis, and if so, what mechanisms are involved.

This appears to be one of the means by which the reproductive system is regulated. Leptin also regulates food intake and other brain processes. Leptin acts on specific cell types in the brain. Some of these may have dual function to regulated appetite as well as reproduction. The present proposal is for work to determine mechanisms within the brain that are altered by leptin. We will also determine which specific mechanisms relate to the regulation of gonadotropin releasing hormone. The work will provide information on how putative appetite regulators might affect the reproductive axis. Such work will provide a platform for design of pharmaceutical means to manipulate the reproductive axis and will impact on the design of drugs that regulate obesity.
