Summary
Home mechanical ventilation (HMV) is a treatment that assists someone to breathe using a machine. People may need HMV when their breathing has failed due to conditions such as chronic obstructive pulmonary disease (COPD), neuromuscular disease (NMD) or severe obesity. This project will examine the quality of life of patients receiving HMV in Australia and Canada. It will also determine if we can improve the treatment by using a sleep study to monitor breathing when patients first start HMV.
Improving Neonatal Transition For Compromised Infants And Minimising Lung Injury

The projects in this proposal are designed to improve the health of newly born infants before they are born (fetal lactate blood test to assist in decision making), at birth (randomized clinical trials to provide evidence for future resuscitation guidelines) and in the first few hours following preterm birth by studying alternative methods of providing existing therapies for neonatal respiratory distress syndrome. These studies may prevent harm and allow a “Healthy start to life”.

The last 2 decades heralded many important advances in the care of preterm infants but important gaps in our knowledge remain. Studies undertaken in the preclinical and clinical settings during this Fellowship will enhance understanding of the complex nature of disease in preterm infants. Further, we will develop and evaluate new treatments that enhance growth and development of the premature baby to optimise their long term intellectual, physical, emotional and social outcomes.

Summary
Effective mask ventilation is the most important intervention at birth that can reduce mortality and disability in term and preterm infants. I will develop strategies to help clinicians improve their resuscitation skills. I will also study new ways to better support babies’ transition after birth, to improve their short and long term outcomes. The results of this research will change the way newly born babies are cared for around the world.

We have recently completed a study examining the prevalence of birth defects in assisted conception infants born in Western Australia from 1993-1997. Contrary to reassuring claims by other researchers in this area, we found that assisted conception infants have a two-fold increased risk of being diagnosed with a major birth defect by one year of age. We now propose to examine other long-term health outcomes in these children. This study involves record linkage between the WA Reproductive Technology Register and four other population-based databases. The prevalence of cerebral palsy, intellectual disability, hospital admission and birth defects in assisted conception children born in WA between 1993 and 2001 will be compared to that seen in all other Western Australian children born over the same time period. The collection of information on risks associated with assisted conception treatment is vital to allow adequate counselling of couples considering fertility treatment. Cerebral palsy, intellectual disability, birth defects and hospital admission are all serious adverse health outcomes and, despite the introduction of IVF to most Western countries twenty years ago, there are limited data in the literature concerning the occurrence of these conditions in assisted conception infants. Quantifying the contribution of assisted conception treatment to neonatal, infant and childhood morbidity and mortality is also important for the planning of health service provision. Although assisted conception births represent only a small proportion of total births in Australia, these infants may require a disproportionate level of health care services, such as neonatal intensive care treatment due to complications associated with preterm or multiple birth. Summary
In this project we aim to establish techniques to obtain viable and developmentally competent eggs from embryonic stem (ES) cells for studies on the molecular and cellular mechanisms of sex cell production. We expect to achieve ES cell derived eggs with similar fertilization and developmental potential as eggs developed naturally. Sterility resulting from cancer treatments and from genetic and non-genetic malformations can benefit from this ES cell therapy.
