Summary
BCG (used till recently to prevent tuberculosis) is a potential low cost and readily available vaccine which could reduce the rates of allergy and infection in Australian children. We propose to extend our existing NHMRC-funded trial, which studies whether BCG vaccinatinon given at birth prevents the development food allergy, eczema and infection in the 1st year of life, to see if this effect continues until 5yrs of age. At this age, we can also see if BCG vaccination at birth prevents asthma.

Summary
This project will investigate the relationship between the bacteria a mother carries in her gut during pregnancy and her baby’s risk of developing allergic disease and asthma. We will investigate underlying immune pathways and conduct experiments in mice to determine whether inoculating mothers with a specific type of bacteria known as Prevotella could be used to prevent food allergies and asthma in the offspring.

Summary
Cystic Fibrosis (CF) is the most common life-threatening genetic condition affecting Australian children. As well as repeated lung infections, children with CF develop insulin deficiency and eventually diabetes. The CF-IDEA trial (Cystic Fibrosis _ Insulin Deficiency, Early Action) will determine whether starting insulin treatment before the onset of diabetes (earlier than current practice) will improve the health of children with CF by improving body weight and lung function.
The prevalence of food allergy (FA) has risen dramatically in the last 20 years. Strong evidence suggests that undetermined lifestyle factors are central to this modern phenomenon. We will develop a CRE in Paediatric FA and Food-related Immune Disorders, providing international leadership in public health initiatives to stem the tide of FA and related disorders. We are uniquely placed to provide evidence-based guidelines that will inform public health policy and clinical care of patients.
Investigation Into Host Susceptibility And Immune Responses In Young Children With Acute Wheezing Due To Human Rhinovirus Group C Infection

Summary
We recently made the surprising discovery that a new viral group, human rhinovirus group C (HRV-C), causes the majority of acute asthma in children. We also found that it causes half of the acute wheezing attacks in younger children, and is the only respiratory virus associated with allergy. So, HRV-C may be the key to the relationship between allergy and asthma. The planned project will focus on whether young children who wheeze with HRV-C have related defects in their immune system.
Prophylactic Antibiotics To Prevent Recurrent Lower Respiratory Tract Infections In Children With Neurological Impairment (PARROT) Study

Summary
We plan a randomised controlled trial to determine if 12 months of a type of antibiotics (compared to placebo) reduces hospitalisations in children with neurological impairment. Currently this group of children are recurrently hospitalised and some doctors use long term antibiotics but there is no high level evidence for this practice. The study will be undertaken in the UK and Australia and involve 474 children. The study will lead to better clinical care and inform guidelines.

As medical advances over the past 50 years have helped to reduce traditional childhood illnesses such as infections, behavioural, developmental and mental health problems have increased. These problems affect at least 1 in 5 Australian children yet the vast majority of problems go undetected and untreated. I propose to develop, trial and disseminate evidence-based approaches to common child health problems including mental health and sleep problems.

Summary
This project seeks to characterize the inflammatory mechanisms triggered during severe respiratory virus infections that result in hospitalization of infants. These are associated with later development of asthma. We aim to identify new molecular targets for anti-inflammatory drug development, with the long term goals of improving treatment for acute disease, and prevention of subsequent asthma.
