Summary
Falls are a major health and injury problem for people with Parkinson's disease (PD). In this study we propose to undertake a randomised control trial to determine the efficacy of textured insoles to improve postural stability and reduce falls risk in people with Parkinson’s disease. The success of such an inexpensive device would result in improved health outcomes for individuals and large savings in health care through a reduction in fall-related injuries.

The aim of my project grant is to develop a portable electrodiagnostic test to classify balance disorders by compiling a statistically validated database of eye movement patterns associated with specific balance disorders. What is unique about this electrodiagnostic balance disorder test, which distinguishes it from but also complement conventional balance function tests, is its ability not only to show an impairment of balance function, but it can differentiate between balance disorders.

How does the brain control balance when we stand? Our research shows that two very distinct processes operate to produce distinct postural, perceptual and cardiovascular outcomes. These experiments investigate the neurophysiology that coordinates these systems and what happens with age. The results will fundamentally change views on balance control. Balance problems are common in neurological disorders and old age. Knowing how balance works will improve diagnosis, treatment and rehabilitation.

Our research aims to identify how specific brain cells detect changes in glucose levels and how ageing and diet affect their function. We identified a subset of nerve cells that detect changes in glucose and the “hunger” hormone ghrelin, their ability to do so adapting with age and nutritional status. Major surgery can result in serious complications, some of which lead to permanent disability and early death. All patients undergoing major surgery require intravenous fluids to maintain a healthy circulation, but too much fluid can cause tissue swelling (oedema) and "drown" the vital organs. There are probable benefits of a restrictive IV fluid regimen. This could have major benefits to patients having surgery.

The rate of falls in older people remains a significant problem. We have developed and validated a home-based computerised training intervention that can be delivered either while seated (cognitive) or while standing and undertaking balance exercises (cognitive+motor). This unique design will allow us to assess whether cognitive and cognitive+motor training can prevent falls, as well as the neural, physiological, physical and neuropsychological mechanisms behind the intervention effects.
