Summary
With an ageing population and associated increase in dementia there will be increased demand for neuropsychological assessment, with insufficient trained personnel and resources to meet this demand. Computerised tests offer excellent opportunities for large scale implementation of cognitive screening and monitoring of older adults. This is the first study to systematically evaluate and compare several popular computerised neuropsychological assessment devices in elders with and without dementia.

We have recently completed a study examining the prevalence of birth defects in assisted conception infants born in Western Australia from 1993-1997. Contrary to reassuring claims by other researchers in this area, we found that assisted conception infants have a two-fold increased risk of being diagnosed with a major birth defect by one year of age. We now propose to examine other long-term health outcomes in these children. This study involves record linkage between the WA Reproductive Technology Register and four other population-based databases. The prevalence of cerebral palsy, intellectual disability, hospital admission and birth defects in assisted conception children born in WA between 1993 and 2001 will be compared to that seen in all other Western Australian children born over the same time period. The collection of information on risks associated with assisted conception treatment is vital to allow adequate counselling of couples considering fertility treatment. Cerebral palsy, intellectual disability, birth defects and hospital admission are all serious adverse health outcomes and, despite the introduction of IVF to most Western countries twenty years ago, there are limited data in the literature concerning the occurrence of these conditions in assisted conception infants. Quantifying the contribution of assisted conception treatment to neonatal, infant and childhood morbidity and mortality is also important for the planning of health service provision. Although assisted conception births represent only a small proportion of total births in Australia, these infants may require a disproportionate level of health care services, such as neonatal intensive care treatment due to complications associated with preterm or multiple birth. Summary
Eggs with too few copies of mitochondrial DNA either fail to fertilise or arrest during early development. By supplementing eggs with mitochondrial DNA, we have been able to enhance embryo quality and gene expression profiles. By breeding the offspring derived from eggs given mitochondrial supplementation, we will determine if they and their progeny meet normal developmental milestones, regulate the transmission of mitochondrial DNA appropriately, and are healthy and fertile.

My research program will use a comprehensive set of linked population data to address key questions in the IVF field following major changes to IVF practice in the last decade. This research (examining fetal growth, birth defects, intellectual disability and school achievement) has the potential to influence clinical practice and will greatly improve the information available for pre-treatment counselling.

The injection of a single sperm into the egg (ICSI) has been the main IVF treatment for men with poor sperm quality since 1993 but is now often used for other types of infertility. Concern has been raised about the health of the children. We will approach >800 parents and their adult sons conceived using ICSI and will assess his general health and development, and fertility. This work will improve patient counselling and practice guidelines, and direct research into the safety of ICSI.

Many women cannot have children because of suboptimal egg quality, often due to aging. Currently, the only option is to use better quality eggs donated from another woman. This project will use pharmacological agents to promote recently discovered pathways in eggs central to determining quality. Importantly, we will investigate a simple and practical approach that can be used in clinics for augmenting these pathways to improve oocyte quality for the first time.

The majority of young people with anxiety disorders do not seek nor receive treatment. This may be due partly to the perceived stigma of attending a mental health clinic and logistic problems of attending therapy. A challenge is to identify ways in which treatment can be made more accessible to adolescents in need, particularly those in rural and remote areas. Contrary ....Anxiety disorders affect around 5-10% of adolescents and are associated with a range of adverse consequences if left untreated. The majority of young people with anxiety disorders do not seek nor receive treatment. This may be due partly to the perceived stigma of attending a mental health clinic and logistic problems of attending therapy. A challenge is to identify ways in which treatment can be made more accessible to adolescents in need, particularly those in rural and remote areas. Contrary to original beliefs, evidence shows that the internet can be used to deliver effective psychological therapies for a range of mental health problems, including anxiety disorders. So far this research has focused on adults, but adolescents may be in a particularly strong position to benefit from these forms of therapy, being experienced users of internet resources. This research investigates the effectiveness of internet-based cognitive-behaviour therapy (CBT) in the treatment for anxiety disorders in adolescents. Recently we have piloted CBT for anxious children that combines clinic and internet therapy, with half the therapy sessions in the clinic and half over the internet. Pilot data show that clinic + internet therapy produced significant reductions in anxiety and is almost as effective full clinic-based therapy. The proposed research builds on our pilot work by evaluating the effectiveness of a more extensive internet-based therapy in which all sessions are completed using the internet and with an older age group. The research also examines the factors that predict which young people will respond most positively to the intervention so that the treatment can be matched to those most likely to benefit from it in the future. The aim is to develop an effective internet-based treatment for adolescents with anxiety disorders that is easily accessed and suitable for use in rural and remote populations, requiring minimal face-to-face therapist contact.Read moreRead less

Summary
In a recent study, using telephone-interviews, we compared the health and wellbeing of 547 singleton young adults born following assisted reproductive technologies (ART), with 549 matched controls. Reviewing their health when they are 22-33 years is possible because of their ongoing interest. We have a protocol in place to measure their cardiac and respiratory function and other aspects of growth and development. Our findings will fill a major knowledge gap about the longer term safety of ART.
