Summary
Internationally, patient safety is a growing concern. Patient harm occurs in 10% of hospital admissions. A million adverse events occur in general practice each year in Australia. Overseas data reports that patients receive recommended care only 50% of the time. We will significantly advance this work by investigating how and why this ccurs, with a focus on the roles of teamwork, safe medication use and the application of information technology to support improved decision-making.
Establishing A Digital Health Foundation For Outcomes-based Diagnostic Excellence, Safety And Value

Summary
Our partnership proposal will lead to the development of a digital infrastructure (involving big data resources and sophisticated decision support systems) as a foundation for outcomes-based research, optimised clinical care processes and translation into policy and practice. The partnership will encompass key research areas involving the quality, safety and value of care in musculoskeletal care, infectious diseases, diabetes care and emergency/intensive care.
E-health can improve the quality, safety and effectiveness of health services. The Centre for Research Excellence in Informatics and E-health will support the design, evaluation and use of e-health systems in 3 areas: monitoring e-health safety using incident monitoring, evaluating consumer e-health safety, and developing next generation evidence-based support tools.

The case for a shift to evidence-based practice, and the substantial economic and health outcome benefits of that shift, have been repeatedly made. Despite the vision, significant barriers to evidence-based practice remain, and the demonstration of a positive role for on-line systems would result in a significant change in strategies for clinician behaviour change. The failure to follow up test results is a major area of patient safety concern. This proposal is based on a collaboration between the research team, the South Eastern Area Laboratory Services and the Australian Commission on Safety and Quality in Health Care. The proposal aims is to improve patient safety by establishing effective, safe test result management systems using evidence-based practice, sophisticated health information technology and through engagement with consumers.
