There are only a few published cohort studies available on occupational exposures in use and applications of lead compounds, despite the widespread use of these compounds in the past 50 years. Since the late 1980s the use of lead compounds in Australia has decreased, but Australian and world lead production has increased. Australia is the world's biggest producer of lead. The precise number of lead workers worldwide is unknown but there are likely to be more than 2 million workers of which many are in developing countries where control of lead exposure is unsatisfactory and occupational exposure limits are considerably higher than in developed countries. Evidence for or against cancer risk with exposure to lead compounds , in particular inorganic lead, will therefore have implications for many Australian and international workers. This is also important for health risk assessment for environmental contamination in the Australian community. In our study we will review the state government archived records of about 8,000 lead workers in New South Wales, Victoria and South Australia, who took part in lead compound exposure surveys and had blood tests in the 1970s, 1980s and 1990s, as part of government lead regulations in force at the time. We will then match the details of these workers against the National Cancer and Death registry data to calculate cancer rates. Colorectal cancer is Australia's most common cancer. It actually has several subtypes that probably have different causes and which may respond differently to treatment . The aim of this study is to see whether risk factors for a newly identified form of colorectal cancer, called CIMP, differ from those for the more common form of colorectal cancer. Knowing this may help us understand better how the disease is caused and so design better ways to prevent it.

Summary
Human papillomavirus (HPV) is the major cause of cervical cancer and the cause of 5% of all human cancers. HPV has recently also been associated with oral cancer, especially in patients younger than 50 years of age. In this project we will investigate how common HPV infection is in oral cancers in Australia. In these patients we will also investigate mutations in genes that have been found to play a critical role to clear persistent HPV infections and how cells respond to HPV infection.
One of the current challenges in public health is to translate the progress from the Human Genome Project into reduced morbidity and mortality from disease. Once genetic defects are characterised, knowledge about the variability in severity of disease in mutation carriers, is important from a public health perspective. Hereditary Haemochromatosis (HH) is a common genetic disorder of iron overload that results in a wide spectrum of disease, varying from non-specific symptoms to severe damage to liver, heart, pancreas and joints from iron deposition. It is easily treatable by regular blood donation, and population-based screening for HH has therefore been advocated. In this study we aim to address gaps in the existing data on HH regarding dietary and lifestyle factors that contribute to the variable clinical picture of HH. The study will be based on the Melbourne Collaborative Cohort Study, a cohort of 31,500 men and women who have been followed for approximately 10 years. Information on dietary and lifestyle factors was collected at initial enrollment, along with a blood specimen. We will test all non-Southern European participants (31,176) for the common HH mutations in the HFE gene and then select a subgroup of 1150 people, including all people with the main genetic defect as well as a comparison group, for further clinical followup. Participants will have genetic counselling and informed consent will be obtained. Participants will complete a short questionnaire and give a blood sample for measurement of iron overload, liver function, and other relevant blood tests, then undergo a brief clinical examination. Results of all tests will be given at a followup visit by genetic counsellor or physician. This study will provide important data on natural history of HH risk factors that influence variability in clinical presentation and the association of HFE mutations with chronic diseases and all cause mortality.Read moreRead less

Despite more than half a century of vaccination, pertussis has remained endemic in Australia, causing epidemic outbreaks every 3 to 4 years. Indeed the incidence has been increasing in recent years with over 11,000 cases notified in 2005.Pertussis is the commonest of all vaccine preventable diseases targeted by the Australian standard vaccination schedule. To control pertussis, appropriate epidemiological surveillance must be implemented to monitor the circulating B. pertussis strains and emergence of any new strains, in particular those that escape the protection by vaccines. This project will use a novel genomic comparison approach to find variation known as single nucleotide polymorphisms (SNP) present around the genome. We will use these SNPs to analyse a large collection of Australian and worldwide isolates to identify those SNPs most suitable for typing and determine the optimal number of SNPs needed for discrimination. A standardized, consistent and reliable typing method for strain identification will be established. We will further develop an integrated method for detecting known variation in the genes encoding the five acellular vaccine antigens. A combined method for strain identification and detection of antigenic variation will then be developed for culture-independent typing, enabling identification of the strain infecting an individual directly from a clinical sample without the need for culture. This study will establish molecular methods for surveillance of pertussis in Australia, essential for further improvement of acellular pertussis vaccines and public health control measures.Read moreRead less

Although most prostate cancers are not fatal, some develop into an aggressive form that is likely to progress and kill. How this happens and how to prevent these cancers, is not well understood. The study will examine the lives of men with aggressive prostate cancer, from the time of conception to adulthood, their family history and genetic characteristics to identify the role of life patterns and genes in the development of these cancers, seeking a basis for their prevention .

If these carriers can be identified they can take preventive measures such as screening to reduce their risk of cancer and death. We will develop a model using data from the Colon Cancer Family Registry (CFR), the world's largest dataset of carriers and non-carriers which has already recruited and genetically tested over 4,000 families from Australasia, USA and Canada. The model will allow clinicians to predict who is a likely be a carrier based so they can be tested for the mutation. We know the risk of cancer is high in carriers, but we don't have precise estimates. We will use the Colon CFR applying sophisticated statistical methods required to answer this question. This data is critical for genetic counselling so appropriate decisions can be made by the patient and the doctor as to what preventive measures to take. We will also use the Colon CFR data to find out what how the carriers who develop cancer differ from those who stay cancer free using their completed lifestyle questionnaires which includes questions on diet, smoking, alcohol consumption, exercise, aspirin use, and oral contraceptive pill use. This project provides important data on the dose-response effects of a known carcinogen. Due to the integrated nature of our already established follow up systems we are in a unique position worldwide to achieve these aims. The dose response relationships between exposure and disease identified by this study will contribute to occupational health and safety exposure standards and compensation policy in Australia. The information on lifestyle factors is appropriate for use in many different health promotion interventions. There is laboratory evidence that chemicals in green tea (poloyphenols) protect against cancer. Epidemiologic studies in humans have generally supported these findings, especially for breast cancer. This project is a crucial stepping stone towards future prospects of a large-scale trial using green tea extract. It will see if the protection extends to leukaemia and bowel cancer, and will identify the genetic makeup of people who are able to benefit the most from green tea polyphenols.
