Large hiatal herniae (HH) often cause debilitating exercise intolerance - the mechanism of which is unknown. This project will define the functional and haemodynamic significance of cardiac compression in patients with large HH, providing new insights into the mechanism of exercise impairment in these patients as well as clinical tools for pre-operative selection of patients likely to benefit most from surgical repair.

Reactive balance training using unpredictable trips and slips can reduce fall rates in older people by 60%, a significant improvement on current interventions. This project will determine the optimal dose to and physiological adaptations underlying reactive balance training. It will also determine how older people with a high fall risk benefit by this training. This study has the potential to enhance clinical practice and assist older people in maintaining independence and quality of life.

Heart disease is a leading cause of death in patients with breast cancer. Physical exercise during cancer treatment has been shown to improve overall survival by reducing future problems with the heart as well as recurrence of the cancer. Despite this, few women achieve the guideline recommended target for exercise. This study will test a Smartphone application called 'BreastMate' which is designed to promote physical activity and heart health in patients with breast cancer.

The interface between exercise, risk markers, atherosclerosis and cardiac function remains an understudied area of great public health importance. Our project will assess the effect of moderate and high intensity exercise on markers of cardiovascular disease (CVD) and cardiac remodelling.
Despite the clear epidemiological evidence that physical activity can reduce breast cancer recurrence and risk, little is know about the mechanisms. The aim of this project is determine the metabolic pathways and immunological effects of exercise in preclinical breast cancer models and in breast cancer patients, and to determine if there are synergistic effects with current systemic therapies.

Summary
The population is ageing and the prevalence of obesity is increasing. The “Baby Boomer” generation has the highest prevalence of obesity of all age groups, hence increasing numbers of older adults (age over 65) are obese, and this is unprecedented. We know that obesity predisposes to loss of physical function and independence in older adults, and may increase the risk of dementia. Our trial will compare multiple treatments for obesity in older persons, including newer treatments.

The sudden death of an athlete is a tragic event with substantial emotional and social impact on the community.Evidence suggests that cardiac screening of competitive athletes can significantly reduce this risk (reduction of sudden cardiac death in Italy by 75%).It is imperative that we obtain data on both indigenous and non-indigenous athletes in the Australian setting to develop a normal range for our population if we are to develop an effective screening programme.

Pulmonary embolism (PE) is a common, potentially life-threatening condition with up to 50% of survivors developing long-term complications, which are associated with breathlessness, functional limitation and risk of premature death if not diagnosed and treated promptly. This project investigates the use of non-invasive scan techniques and exercise testing for early detection of these complications. The project will also explore new drug targets to treat these complications.
