Multiple sclerosis is the second most common cause of neurological morbidity in young Australians after trauma. Knowing who will progress to develop multiple sclerosis after a first attack and at what rate they will progress is an important question as it will allow us to target treatment to those at greatest risk and modify a person's lifestyle to reduce the risk of developing MS or slow their rate of progression.

Cancer patients represent some of the more poignant and in need groups. Of this patient cohort, young persons with cancer represent a group with potentially, and understandably, a wide range of high unmet needs. The stress and demands imposed upon parents who have children with cancer is acknowledged as being considerable. Despite the development of measures to identify unmet needs of various cancer patients including advanced, incurable cancer, breast cancer and cervical cancer, there has been no work to date on developing an accurate measure of unmet needs among parents of young persons who have cancer, as well as the unmet needs of young persons themselves who have cancer. This study will develop such a measure, as a first step in identifying the most cost-effective ways to address the unmet needs of children with cancer and their families. We will seek to address an important clinical problem in orthpaedics, namely the bone loss that commonly occurs around joint replacement prostheses. Termed peri-prosthetic osteolysis (PO), this bone loss can result in the loosening and ultimate failure and need for revision of the artificial joint components. PO is thought to be caused by the body's reaction to wear particles generated from the articulating surface of the prosthesis. However, it has not previously been possible to accurately exp ....We will seek to address an important clinical problem in orthpaedics, namely the bone loss that commonly occurs around joint replacement prostheses. Termed peri-prosthetic osteolysis (PO), this bone loss can result in the loosening and ultimate failure and need for revision of the artificial joint components. PO is thought to be caused by the body's reaction to wear particles generated from the articulating surface of the prosthesis. However, it has not previously been possible to accurately explore the relationship between prothesis wear and PO, or the progression of PO, because of a lack of techniques to image and measure the volume of PO around metal prosthesis components. We have developed a means to accurately and reproducibly measure the volume of bone loss, using CT, and will do so longitudinally in joint replacement patients to obtain the first information about the progression of PO. New computer based methods will be used concurrently to relate prosthesis wear and migration parameters to PO. Patients who come to surgery for replacement of failed prostheses will be investigated further by analysis of the tissues involved in the bone loss around prostheses. Basic science experiments will seek to understand the underlying causes of PO and the findings will be important in interpreting the clinical results. An animal model will be used to seek approaches to inhibiting the pathological response to wear particles. The significance of these studies is that they will lead to improved outcomes for joint replacement patients, increasing the interval to revision surgery, which is both extremely costly and brings an attendant morbidity and mortality.Read moreRead less

Assessment of the effect of changes in diet of health has not undergone rigourous analytical assessment in the scientific literature. This proposal focusses on patterns of change over time in a Mediterranean diet in 41,000 persons in the Melbourne Collaborative Cohort Study. Summary
In multiple sclerosis (MS), permanent disability occurs when brain cells known as neurons are damaged following an immune attack. Current treatments reduce the number and severity of immune attacks, but they do not prevent neuron damage or permanent disability in many patients. There is currently no direct way to measure neuron damage in humans, so it is difficult to develop new drugs to prevent it. To address this need, we will trial a new blood test for measuring neuron damage in MS patients.

Summary
75% of the mortality in adults with Type 2 diabetes is due to a cardiac event. Early detection and treatment of cardiac disease is paramount in improving health outcomes. An echocardiogram is an accurate and non-invasive identification of cardiac dysfunction. We will assess the prognostic value of echocardiography and novel plasma markers in patients with diabetes. Our results may lead to new management and screening guidelines for heart disease in diabetes.

There is good evidence that population screening for bowel cancer (CRC), based on the detection of blood in stools, is effective in reducing deaths from bowel cancer by around 30-40%. Screening depends on the use of a simple test to identify those who most need the complex and costly test which is capable of accurately detecting curable cancers and precancer lesions. This can be achieved with moderate effectiveness using simple tests (FOBTs) which detect microscopic amounts of blood in the faece ....There is good evidence that population screening for bowel cancer (CRC), based on the detection of blood in stools, is effective in reducing deaths from bowel cancer by around 30-40%. Screening depends on the use of a simple test to identify those who most need the complex and costly test which is capable of accurately detecting curable cancers and precancer lesions. This can be achieved with moderate effectiveness using simple tests (FOBTs) which detect microscopic amounts of blood in the faeces. If we are to reduce the rate of death from CRC, we must have an effective way of encouraging people to do these tests. While much has been learnt from experience with screening for breast and cervical cancer, CRC presents a series of quite different issues that have never been comprehensively studied. These are: (1) men and women need to be screened. (2) symptoms due to CRC are more complex , (3) the high-risk settings for CRC are much more complex), (4) the community is not as aware of the benefit of screening, (5) the initial test can be performed at personal convenience in one's home, (6) participants must handle bodily excretions, and (7) inconvenience of attending a central facility is avoided. We will survey participants and non-participants to more accurately identify the barriers to screening, and the proportion who have not participated for informed reasons. To test the real value of attempts to overcome these, we will then offer screening by various approaches designed to overcome these. We are in a unique position to do this as we have well-identified populations who have been offered faecal occult blood test (FOBT)-based screening These studies will assist in the design and implementation of effective screening programs for the early detection of CRC in Australia, which in the long term will significantly reduce deaths from this disease.Read moreRead less
