In exposure therapy, the patient, aided by the clinician, confronts trauma-related cues in the absence of any overt danger. The intention of this therapy is to reduce the ability of the trauma-related cues to provoke the fear reactions that are undermining the patient's quality of life. In Pavlovian conditioning, subjects (typically rats) are exposed to a signaling relation between an initially neutral stimulus (e.g., a noise) and a feared outcome (e.g., foot shock). When later repeatedly exposed to the initially neutral but now feared stimulus (the noise) in the absence of the feared outcome, the fear reactions it acquired progressively decline until eventually it fails to elicit any such reactions. The fear reactions are said to have been extinguished. There has been significant progress in understanding the psychological processes and neural mechanisms underlying the acquisition of fear reactions, but much less is known about the processes and mechanisms underlying the extinction of these reactions. The project has two general objectives. The first is to determine the conditions of extinction training that promote long-term loss of fear reactions. The second objective is to determine how the brain controls this extinction of learned fear. Achieving these aims will be significant for two reasons. First, it will contribute to understanding the mechanisms by which animals (including people) learn to adjust their behaviour to bring it into line with the current relations that exist between events in the world. Second, it will provide important information about how such adjustment is facilitated or impaired across extinction training and, thereby, contribute towards understanding both the successes and failures of cue exposure therapy for fear-related disorders.Read moreRead less

Summary
Schizophrenia and autism are mental health problems that often occur together. Despite this, little is known about the overlap between them and how to treat people who experience both. In this study, I will compare young adults with schizophrenia and autism using three measures of brain function. I plan to employ complex statistical analyses to better understand the similarities and differences between the disorders in order to improve diagnosis and treatment.

Summary
The long-term illness course and outcomes of patients treated for first episode psychosis are poorly understood especially in terms of important domains such as social and vocational functioning, physical health, and quality of life. This treated cohort study of a sample of 786 patients, 15 years after their first episode of psychosis will be one of the longest and largest conducted to date. Results of the study will inform clinical practice and policy development.

Summary
Our ability to predict the onset and outcome of schizophrenia and other psychotic disorders remains limited. This restricts the implementation of effective preventative and early interventions. This project will rectify this by developing a tool of significant clinical utility. The project has 2 aims: 1. To develop and validate a neuroimaging-based tool for use in early psychosis to assist with early diagnosis, outcome, and disease progression. 2. To apply the tool in clinical practice.
Wernicke-Korsakoff syndrome is a common and frequently undiagnosed neurological disease due to thiamine deficiency. The condition leads to cognitive and neurological disability and mostly affects people with alcohol dependence. Wernicke-Korsakoff syndrome is eminently treatable with thiamine, although we have insufficient knowledge regarding the best dose of thiamine to treat the condition. Testing theoretical models of age and disease related changes to inform prevention. Pathological brain changes associated with future cognitive decline become detectable in the 40s or earlier. Yet little is known about what constitutes normal brain ageing in mid-life. Using a number of neuroimaging and epidemiological techniques this project will scrutinise brain and cognitive ageing in middle-age and their significance.

People with serious mental illness die up to 25 years before their peers, mostly from cardiovascular disease. Despite extensive research, we have made little impact on addressing this health inequity. By exploring the experience and meaning of personal cardiovascular health to people with severe mental illness, this study aims to generate new knowledge that will inform the tailoring of cardiovascular health promotion interventions that meet the unique requirements of this vulnerable population.
