Summary
The health of Indigenous people is poor and a number of risk factors contribute to lower life expectancy and the development of 'aged care syndromes' such as falls, incontinence, and dementia at a younger age. The research team has described high levels of dementia in a population survey completed in 2005 on 363 older people from the Kimberley. This population will be re-surveyed to document ageing syndromes and determine factors that may improve health for this population at risk
Understanding Frailty In Older People: An Epidemiological Insight

Summary
We need to find better and more efficient ways to identify older people at risk of poor outcomes. The measurement of frailty, a key concept in geriatric medicine, may provide this exact information. The proposed research will investigate frailty in large-scale population datasets across both community and hospital settings. Results can be used to guide patient management, influence public health policy and inform future intervention studies.
Hip fractures are a devastating consequence of injurious falls, resulting in reduced quality of life, reduced independence and further falls. The cause of a fall is often multi-faceted. Falls risk assessment tools identifying falls management strategies are available, yet hip fracture rehabilitation approaches often fail to identify and implement these targeted management strategies. The aim of this project is to improve falls and injury risk assessment and management following hip fracture.
Emerging from the shadows: the evaluation of intervention strategies to reduce social isolation amongst the aged. Social isolation imposes a high cost on affected individuals and the community at large. This project will determine what interventions work in addressing social isolation amongst the older population.
The rise of the consumer: engagement of older people with consumer directed care. Australia is adopting a new model of care for older Australians - consumer directed care. This project will provide the evidence needed by governments and service providers to create the most effective and supportive system.
Increasing advance personal planning by older adults. This project aims to increase the uptake of advance personal planning among people aged 65 years or over in the community by developing, implementing and evaluating a community action model. People have a legal right to engage in advance personal planning, a process that helps them discuss and document their financial, personal and health preferences, in case they later lose the ability to make or communicate decisions. Outcomes relate to the individual consumer's needs and require monitoring to ensure actual needs based on thorough assessment are being met. Older adults who participate in social activities benefit from enhanced wellbeing, but social groups are often inaccessible for those in advanced old age, especially if living in rural areas.
