We hypothesised that these infants possibly had a maturational delay of breathing control during sleep. This project is designed to examine the development and prevalence of sleep and breathing disorders in infants. The prenatal factors that possibly influence development of sleep and breathing disorders in infants, in particular, the effects of maternal smoking will be determined. Pregnant women will be recruited for the study during their third trimester. The foetal movements, foetal breathing movements, heart rate and sleep state will be monitored continuously overnight in the patients home between 32 and 36 weeks gestation using a newly developed foetal movement monitor. The infants will be subsequently studied using overnight polysomnography at 2 months of age to assess their breathing, sleep patterns, arousal behaviour, and the presence and severity of central and obstructive apnea. A group from these infants will be selected and studied longitudinally to examine the development of sleep and breathing disorders more closely. These infants will undergo overnight sleep studies during the first week of life, then at 2 and 6 months of age. A detailed medical history will also be collected regarding the pregnancy, the perinatal history of the infant, exposure to cigarette smoke during pregnancy and postnatally, and the medical history of other family members. Summary
During sleep, patients with heart failure often exhibit a cyclic pattern of breathing in which a period of regular breathing alternates with a period when breathing ceases. This breathing pattern is associated with increased mortality and reduced quality of life but to date no effective treatment for it exists. We have developed a method to calculate an exact level of carbon dioxide to stabilise breathing in each individual, offering the prospect of a better night’s sleep and a longer life.

Summary
My primary research focus is to understand the mechanisms underlying sleep-related breathing disorders, such as obstructive sleep apnoea, and develop better ways to diagnose and treat them. I do this by undertaking university-based epidemiology studies in a large sample of community participants, by undertaking smaller hospital-based physiology studies and by working with US and Australian start-up companies to design and test new treatment for sleep disorders.

Summary
Insomnia is highly prevalent and predicts many other mental and physical problems. While very good behavioural treatments exist for insomnia, not everyone is able to comply with the difficult behaviour changes required to improve. Bed partners often influence sleep behaviours, and thus, can play a helpful role in the treatment of insomnia, if they are taught how. We will test the first intervention ever developed integrating the partner into insomnia treatment as a means of improving outcomes.
Most deaths from non-illicit drugs are caused by prescribed opioids and are nearly always due to respiratory arrest during sleep. However, no study has assessed the effect of opioids on vital respiratory drives during sleep. In addition, sleep apnea has been identified as a major risk factor for postoperative morbidity and mortality, while no study investigated how to identify those sleep apnea patients most at risk of opioids. Our proposed studies aim to address these gaps in knowledge.

The amount of time we sleep is maximal in infants and children because it is promotes normal brain development. Sleep problems affect nearly half of all children and adversely affect both learning and blood pressure. The proposed studies will identify and improve treatment of sleep problems to improve overall physical health and quality of life for these children and their families.
